We have a vacancy for a Band 7 Team coordinator in our older adult CMHT in Spalding. This post is suitable for a registered mental health nurse/registered AHP who is able to evidence their experience of people and team management. The post is Monday to Friday 9-5.

The team supports patients over the age of 65 who present with serious mental illness. We also provide an all-age memory assessment and monitoring service.

The team consists of a Consultant Psychiatrist, a specialist doctor, senior mental health practitioners, Associate practitioners, and a dementia support service.

The expectation of the role is to coordinate and manage the patient journey from referral to discharge, ensuring patients and carers are provided with a person-centred, compassionate, and effective experience throughout.

You will be expected to triage and manage referrals effectively following the pathways in the division. You must be able to manage waiting lists and other KPI’s effectively, analyse process and manage data provided on the teams' performance. You will be leading an experienced staff team who have the highest regard for carer and patient experience. Strong leadership and role modelling skills are essential.

You will be part of a supportive divisional management structure, and a Band 7 peer support group. We also have 2 highly functioning home treatment teams within the division who provide intensive support to patients in their own home whilst working alongside CMHT staff.

Spalding adult and older adult CMHT’s are in the second wave of the Community Transformation roll out. Local Integrated Place Based teams are being developed to support those patients with serious mental illness who do not need to be in receipt of secondary services. The pathway is developing a step up, step down approach to ensure a smooth passage for patients through services at differing times of need, with a “no wrong door” philosophy

The Older adult CMHT is based at The Johnson Community Hospital in Spalding. This is a modern purpose-built building on the outskirts of Spalding with easy access to the surrounding road networks. There is ample free parking at the site.
