This is an exciting opportunity to join the well respected Memory Service in South Gloucestershire as a Memory Nurse.

We are a small, friendly and supportive team of practitioners from a range of professional backgrounds who bring different interests, skills and specialisms to the team, whilst sharing a passion for offering high quality, safe and effective assessments and diagnosis for people who are experiencing difficulties with their memory.

We also provide individual and group therapeutic interventions including Cognitive Stimulation Therapy, Life Story Work and Post-Diagnostic Support and Advice.

We work closely with colleagues from other organisations to ensure those accessing our service receive appropriate person-centred interventions and support.

Main duties of the job

You will participate as a full member of the multi-disciplinary Memory Service team.

You will be responsible for coordinating the service user's journey through their memory assessment to diagnosis.

You will play a key role in preparing the service user and carers for the diagnostic process and in seeking consent from the service user regarding further assessment as appropriate.

You will be responsible for completing the full baseline assessment and completing the collateral history with the carer, along with providing ongoing information and support to the service user and carers throughout the length of the assessment.

Job description

Job responsibilities

To provide initial specialist assessment for individuals experiencing problems with their memory and cognition.

Holding a caseload of service users whilst the process of their assessment and diagnosis progresses.

To liaise and participate with other members of the multidisciplinary team, family members, carers, Primary Care and other agencies involved with individual service users to formulate diagnoses.

To communicate diagnosis to individual service users and those in their care network, both in person and in writing.

To be involved in a range of interventions offered by the service, including Cognitive Stimulation Therapy (subject to the necessary training).

Signposting service users and carers to other services as appropriate.

To provide support and advice to Carers through individual and/or group work.

To contribute to integrated governance processes, including the continuing development, improvement and monitoring of service provision and performance, and monitoring standards of care, through data collection and audit processes.

To work in partnership with other local statutory and voluntary organisations in the provision of a range of interventions and support for service users and carers.

To provide support, advice and training as appropriate to other services and agencies involved in the provision of services to people with memory problems.

To be responsible for the supervision of identified staff and provision of training for students of various disciplines as appropriate.

Education & Qualifications

Essential

  • Registered Nurse with NMC (RMN, RGN, RNLD)


Experience and Knowledge

Essential

  • Substantial post-registration experience as a practitioner working with older people experiencing memory problems.
  • Demonstrates a detailed understanding of dementia, including aetiology, symptoms and impact on those effected.
  • Experience and knowledge of using memory assessment tools.
  • Working knowledge of memory enhancing medication.
  • Working knowledge of current developments in dementia care.
  • Experience of working in partnership with other statutory and voluntary organisations.
  • Computer literacy including the use of Microsoft Office, Outlook etc.
  • Computer literacy including the use of Microsoft Office, Outlook etc.


Desirable

  • Experience of setting up and running therapeutic groups.
  • Experience of providing training to others on issues relating to memory issues and dementia.
  • Experience of providing supervision to delegated staff.


Skills and Abilities

Essential

  • Able to prioritise and manage own workload when faced with competing demands.
  • Demonstrates a sound understanding of clinical risk assessment.
  • Able to develop therapeutic relationships in which people using services are empowered and enabled to make informed choices about their health and healthcare.
  • Demonstrates an understanding of the importance of equality, diversity and inclusivity in our work.
  • Able to work in a team and independently.
  • Self motivated with drive and enthusiasm.
  • Excellent verbal and written communication including interpersonal, presentation and liaison skills.
  • Must be willing/able to undertake all appropriate Trust mandatory training requirements, and subsequent refresher training relevant to the area.

Essential

  • Empathy for individuals who have experienced mental health problems and ability to cope with behaviour which may be challenging at times.
  • Ability to travel around the geographical area covered by this post.
