Area West Older People's Mental Health (OPMH) Team for Continuing Healthcare Care have an opportunity for an enthusiastic and motivated Registered Nurse to apply for the role of Continuing Health Care Matron.
The post holder will act as the professional lead providing clinical advice, leadership, development, monitoring and management of all nursing resources for Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) within Area West. The role delivers leadership and quality assurance on the National Continuing Healthcare Framework against patient's eligibility for Continuing Health Care and Nursing Care (FNC).
Main duties of the job
This role requires the planning and commissioning of appropriate care packages to meet patient's needs, monitoring that the care packages continue to meet their needs and review the planned care to ensure it provides a high standard of quality and safety.
The role provides operational CHC team support in the Area West across the local Community Resource teams (CRT) including both NHS hospitals, the independent sector Care Homes and domiciliary care agencies. Delivering a commissioned CHC Service for both general and OPMH patients, this role involves working closely with the CRTs. In addition, the role has oversight of quality monitoring, escalating concerns, safeguarding, clinical incidents and concerns within the CHC service.
Working within Continuing Health Care is a challenging role and involves dealing with sensitive information and skilled communication is a vital part of the role. There is an expectation that the candidates will have knowledge of the Continuing Health Care Framework and understanding and experience of assessing patients for continuing health care.
The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.
