An exciting opportunity has arisen for an innovative, enthusiastic and motivated Neurology Nurse to join our experienced and friendly Community Neuro Rehabilitation Team, which includes an integrated Stroke Early Supported Discharge (ESD) service.

You will be based in the heart of Richmond a beautiful part of London. There will be a high proportion of home visits. You will also see outpatients in our bright, modern premises, which has a supportive reception team and space to upload patient records and complete other admin tasks.

You will provide a nursing service to Stroke patients on discharge from hospital, including medication reviews, secondary prevention education, information and advice. You will receive excellent support from the wider team which comprises Neuro therapists.

You will also develop skills in Multiple Sclerosis and Parkinson's by working closely with, and supporting, the experienced specialist nursing team.

No weekend or on call work. Free parking.

Main duties of the job

You will have clinical responsibility for the care and management of patients following a Stroke and work closely with family and carers. You will also support the specialist nursing team to provide support and reviews for people with Multiple Sclerosis and Parkinson's.

You will see patients in their own homes, in our outpatient clinic and/or at other appropriate settings to facilitate rehabilitation.

You will be part of a dynamic MDT consisting of Specialist Therapists and Nursing.You will participate in delivering education groups for people with Stroke, PD and MS, and secondary prevention advice for people following Stroke. You will be involved in reviewing medication for patients at home and following hospital discharge.

The whole team contributes to ongoing clinical development and improvement projects to ensure that we provide an excellent service for our patients. Continuing personal development is encouraged through training opportunities and an established supervision and appraisal structure. You will be supervised by an experienced band 7.

Supervision of junior Therapists, Therapy Assistants and Students as required.

You will be required to travel within the borough of Richmond, therefore ideally you will be a car owner / driver. A keen cyclist would also be considered.

An informal visits to Richmond Rehabilitation Unit is encouraged.

Person Specification



  • Registered with NMC


  • Specialist qualification in neurological nursing or a commitment to undertake a course



  • Basic theoretical and clinical knowledge of neurological conditions
  • Examples of evidence based practice in the clinical setting


  • Knowledge of the workings of community nursing services
  • Involvement / experience of audit, research and understanding quality issues



  • Considerable post-graduate experience working in an NHS setting
  • Experience and understanding of multi-disciplinary team working


  • Relevant neurological nursing experience
  • Experience of contributing to clinical education of less experienced staff, students or assistantst

Skills and Abilities


  • Very good patient assessment skills
  • Able to handle complex or distressing issues sensitively with patients, carers and families
  • Able to work as an autonomous practitioner and refer on / escalate as appropriate
  • Able to present information, either written or oral, in a clear and logical manner
  • Able to manage time and organise, prioritise and delegate work
