We are recruiting for a Team Lead Dietitian - Frailty and Acute Medicinewho shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.

As part of our forward-thinking dietetic service we have an opportunity at Medway Hospital for an enthusiastic Dietitian to lead our team working in our acute medical, emergency department and front door assessment areas, including following patients through to frailty and care of the elderly wards.

Main duties of the job

This unique role will involve establishing a nutritional care plan for our patients as early as possible, providing enhanced level knowledge and facilitating pathways.

Job description

Job responsibilities

As an autonomous practitioner, to provide advanced level dietetic care to an agreed caseload of patients focusing on leading a team covering early assessment and emergency department areas, acute medical, frailty and care of elderly wards and providing enhanced level knowledge and opinion, across a variety of acute specialities when required.

To take an active role in developing the dietetic service within this area to raise awareness on the importance of early nutrition intervention with education sessions, ward presence, participation in ward rounds and multidisciplinary meetings to provide a coordinated, evidence based approach to patient care and facilitate pathways with the support of the Clinical Lead Dietitian and MDT

To work effectively as part of an Acute Dietetic Team undertaking advanced assessment of patients including those with diverse and complex presentations and conditions: using expert clinical reasoning skills and assessment techniques to formulate individualised treatment plans and programmes

To be professionally and legally accountable for all aspects of own work including responsibility of an allocated patient caseload / workload

Person Specification

Qualifications

Essential

  • Diploma/degree in Nutrition and Dietetics
  • Current Health Professions Council registration
  • Member of British Dietetic Association, ideally including relevant specialist group


Knowledge

Essential

  • Competency in specialist level nutritional assessment of patients in varied clinical specialities in a range of population groups
  • Demonstrates knowledge and understanding of the issues involved with working in a busy acute setting, in particular assessment areas, such as the Emergency Department
  • Awareness of frailty scores and complexity of comorbidities in this patient group
  • Practical knowledge on long term feeding tube care including management in an emergency care setting


Experience

Essential

  • Demonstrable experience of working with a variety of clinical specialities and a range of population groups
  • Experience of supervising Dietitians and Dietetic students
  • Involvement of developing department and service protocols
  • Team lead experience


Skills

Essential

  • Excellent written and verbal and non-verbal communication skills
  • Able to communicate clearly with other team members and patients/ carers in challenging situations
  • Auditing skills
