Band 7 Senior Practitioner with the South Gloucestershire Later Life Community Mental Health Team.

The CMHT and Therapies for Later Life service have an interesting new opportunity to work with our evolving and passionate multi-disciplinary team as a Band 7 Senior Practitioner. You will work closely with the Team Manager and Senior Practitioners to provide leadership for the multi-disciplinary team. The team works with older people who have severe mental health problems, both functional and organic illnesses.

Main duties of the job

This role will give you the opportunity to develop your leadership and quality improvement skills. You will be required to carry out assessments, and develop existing skills in specialist assessment and interventions. You will need to be proactive, a good problem solver, have excellent clinical standards and experience in Supervising members of the clinical team. You will demonstrate an ability to develop therapeutic relationships with people in a range of challenging situations and demonstrate sound positive risk taking and clinical reasoning skills. You will need excellent communication skills and be flexible and resourceful in your approach. We are looking to you to help develop our evidence based clinical pathways You will need to demonstrate your ability and commitment to team working and a genuine interest in achieving wellbeing and positive health outcomes for the older adult population.

Job responsibilities

Key Local Information :

The Community Mental Health Team for Later Life and the Later Life Therapies Team work within the Later Life Teams based at Kingswood Civic Centre. The Later Life services also includes the South Gloucestershire Memory Service and the South Gloucestershire Specialist Care Home and Enhanced Dementia Service. The CMHT and Therapies teams are non-integrated teams and work closely with the Local Authority in South Gloucestershire.

We are a secondary service working predominantly with service users 65 and over with functional or organic illness. Referrals come from the Primary Care Liaison Service, Mental Health Liaison Teams, ARRS practitioners based at GP practices, the South Glos. Intensive Support Team, mental health in-patient wards and other teams within the service.

Our Multi-Disciplinary team includes Community Psychiatric Nurses, Occupational Therapists, Consultant Psychiatrists and Specialty Doctors, a Physiotherapist, Community Support Workers, a Mental Health and Well-being Practitioner and a Psychologist and Assistant Psychologist.

We cover a large and diverse area across South Glos and we are divided into two patches, East and West. We work closely with the Primary Care Networks in our areas.

This is a new role which has been created from vacancies within the Community Mental Health Team and the Later Life Therapies Team. The role will involve working closely with Team Managers from Therapies and CMHT, and the Senior Practitioners, to provide clinical leadership. The role will involve service development supporting the team to meet the challenges of the Community Mental Health Framework, and planned changes to ways of working, including Your Team Your Conversation Your Plan which will be replacing the Care Programme Approach in 2024. The clinical role will also involve carrying out complex assessments and carrying a small caseload. There will also be some delegated management responsibilities to support the Team Managers.

Person Specification

Registration in appropriate discipline

Essential

  • Registered Mental Health Nurse, Social Work qualification, or Allied Health qualification
  • Current registration
  • Substantial post registration experience undertaking assessments and delivering therapeutic interventions.s
  • Significant experience in complex assessments, risk management, and care planning


Desirable

  • Experience of clinical leadership
  • Relevant post- registration qualifications
