B5 Registered Nurse rotational post with the discharge department
Two parts of this involve
1To work with the Integrated Discharge Nursing Team. The post holder will be given training and will work with the Integrated Discharge Team and the ward staff to ensure the patient is discharged from hospital using the Discharge to Assess / Home First Model
Main duties of the job
Integrated Discharge Team- work with ward staff and Integrated Discharge team to discharge patients from hospital as soon as they clinical stable using the Discharge to Assess / Home First Model. Involves visiting wards daily

Working as part of the Discharge planning team to ensure discharge plans start from when the patient is admitted to hospital.
To work within the Integrated Discharge Team (IDT ) under the supervise of the B6 Senior Nurse using the Discharge 2 Assess / Home First Model as soon as it is clinical safe to do so.
This will involve daily visits to adult wards, offering advice and support to patients, families and carers.
Supporting ward teams to facilitate safe and timely discharge from hospital.
Will be responsible for a patient case load under the supervision of the Band 6 Discharge Nurse.
Will attend Board Round and Ward Rounds. Speaking to patients, relatives, carers NOK and to all members of the MDT to ensure the patients/relatives are fully consulted through-out their hospital stay.
Will inform patients and families that it is there responsibility to ensure the home is suitable for the patient to return to eg heating and food is available. The patients also have door clothes to return home
Will discuss with the patient NOK that the expectation is that patients will be discharged home from the Discharge Lounge with their own transport. Only patients who are eligible for NHS free transport will be provided.
To be responsible and accountable for the delivery of safe, effective and compassionate nursing care for patients designated to their care.
To always work to the NMC Code.
To supervise non-registered staff as stipulated by the NMC under the requirements of delegation.
Provide effective leadership and assist with the management and organisation of the ward/department in the absence of the Ward Manager or Junior Sister / Charge Nurse/Nurse in charge.
To comply with the trusts vision, values and behavioural compact.

Qualifications
Essential
- Current NMC Registration (Level 1 adult nurse) or evidence of qualification if newly qualified.
- Diploma in health related study or equivalent appropriate qualification.
Desirable
- Degree in nursing studies or healthcare related subject (UK equivalent).
- Mentorship certificate/ Practice Assessor training or demonstrable equivalent experience.
Experience
Essential
- Experience of working in an acute hospital or community healthcare setting.
Desirable
- Recent experience of working as a registered nurse.

- Ability to provide and receive complex, sensitive or contentious information.
