Applications are invited from highly motivated and experienced nurses with a demonstrable track record of practical application and theoretical knowledge of the care and treatment of people with complex mixtures of psychological, cognitive, functional, behavioural, physical and social risk usually related to mental disorder and frailty experienced in later life. There are 3 posts available to cover Carmarthenshire, Pembrokeshire and Ceredigion.
The post holder will be expected to represent and lead on the OACMHT developmentof alternative pathways (holistic multi-agency and multidisciplinary) for people living with diagnosed/undiagnosed dementia in care homes within the locality, experiencing acute-frailty distress.
The post holder will be responsible for the coordination and delivery of rapid response for urgent face to face assessment of those patients registered to the OA-CMHT.
The post holder will lead on the provision of Dementia diagnostic and treatment clinics for complex older people living in care settings, to seek out and assess their mental health conditions, in order to inform the diagnostic process and to support subsequent interventions and treatment regimens and evaluation of that planned care.
We would be willing to offer this post as an Annex 21 development .Main duties of the job
The post holder will work to agreed job plan, which will include holding a small caseload of clients subject to CTP, undertake caseload management and running clinics primarily within the county care home sector or across the service area as the need arises.
The post holder will undertake a full holistic assessment including the review of medicines, specifically Acetylcholinesterase Inhibitors and Anti-Psychotics for those diagnosed with Dementia within the locality care home sector promote patient choice and effective outcomes.
The post holder will be able to provide senior knowledge and experience within the team and take a senior position in the Multi-Disciplinary Team (MDT) to formulate & implement care plans and consider prescribing options.
The post holder will be involved with key areas of practice and service development, and strengthen links with partner agencies in mental health and general care settings.
To accept clinical responsibility for a designated workload of specialised patients, and to organise this effectively. They will offer expert assessment of psychological, social problems and as an independent prescriber have an in-depth expert knowledge of the physical problems related to the medicines used in treatment.
To undertake non-medical prescribing in line with legislation. To titrate medication dosage, order and interpret physical investigations such as blood tests and ECG, administer injections and to perform Phlebotomy tests on a daily basis.About us

This post is advertised as a Band 7, however applicants who do not currently hold all the necessary competencies to a Band 7 level are encouraged to apply and, if successful, will be appointed under Annex 21 rules whereby 75% of the top of Band 7 will be payable during the 12 months. Applicants would be supported to develop their competencies required to fulfil the role within 12 months. Following successful completion and successful attainment of skills and competences for the role, the post holder will then be paid at band 7 (minimum point on the scale). Failure to achieve the competencies in the designated timeframe could result in re-deployment.

- Registered Nurse (Mental Health).
- Masters level degree or equivalent relevant experience.
- Evidence of ongoing professional development in the delivery services to people with living with Dementia.
- Skills in interacting therapeutically with service users who have a range of presenting problems including BPSD.
- Knowledge of relevant DoH, clinical and NICE guidelines
- Skills in collaboratively assessing, planning, providing and evaluating programmes of care.
