Wallasey and West Wirral Community Mental Health Team are looking to recruit a Band 5 Community Mental Health Practitioners. The role will focus on providing support to the team in delivery of safe and effective clinical service, the main aspects of the role is medication review and administration, assessment of mental and physical health. The work involves working within a clinic or community setting. The post holder will hold a Registered Mental Health Nurse qualification. They will work under the direction of the Clinical Lead, have good communication skills, an ability to work in partnership with service users and carers and be able to work within a multi-disciplinary team setting.
We are looking to recruit enthusiastic, self-motivated team player, who is able to support and be actively involved in the further development of integrated working practice within the team.
Main duties of the job
1. Manage individual casework in the most efficient manner possible, taking into consideration the economies of time, distance and expense whilst maintaining the highest standards of care.2. Liaising, consulting and discussing all aspects of community mental health nursing.3. Participate in weekly multidisciplinary meetings.4. Maintain recording systems to support the above, providing statistical information to agencies and the Team Manager/head of clinical services as required.5. To manage a defined caseload with responsibility for the review of care needs for service users.6. Delivering professional standards of care using evidence based practice.7. Ensuring that all communications with colleagues, clients and families are clearly understood, giving written confirmation of verbal communications where there is observed to be a need.8. Promote the recovery agenda with service users9.Taking responsibility for maintaining and advancing own professional knowledge and keeping abreast with current developments.10. Being responsible for checking and recording prescribed medication, in accordance with the established procedure.11. The post holder will be expected to carry out any other duties appropriate to the role that is requested of them by the team manager / clinical lead / head of clinical services.
