We are looking for a flexible,innovativeand motivated advanced practitioner to support the deliveryof our strategic aimto prevent unnecessary escalation of care where possible, byprovidingproactive support, and to enable access to the right care, first time for people who have a need for urgent or emergency care.
As pressure on emergency departments continues to grow, new care modelsand ways of workingare pivotal in relieving pressure through admission avoidanceandfacilitatingsafe early discharge.
We are offering an exciting opportunity for highly motivated, dynamic,and inspirational individual to join ourteam to help lead and shape our care models working with similar minded clinicians about the system.
We would welcome applications from practitioners with experience and with an advanced practice qualification atMaster'slevel or working towardsMaster'slevel.
In this role you will be supported by a team of senior clinicians to provide ongoing personal and professional development.
Main duties of the job
Contribute to the design/delivery of urgent primary care and intermediate care services, aiming toprovidesafe andappropriate care at home aligned to the Health Board's Urgent and Emergency Care programme and its agreed principles/ scope
Work in partnership with the Urgent and Intermediate Care and WAST multidisciplinary team toprovideremote clinical triage and assessment to patients who have presented to WAST and whose conveyance to hospital could be avoided
Arrange referrals to community pathways (eg mental health services, intermediate care crisis response, patient's own GP), including scheduling of appointments in emergency units and SDEC
Work with key multi agency team members to support the development of a coordinated clinical streaming hub
Develop and strengthen interprofessional working between key stakeholders in Urgent primary/ emergency care to support the development of a place-based county-wide clinical response team
Support opportunities for innovation and implementation including the development of a community diagnostics strategy,providingcare closer to home
The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.
