Our Urgent Community Response Service provides urgent care and therapy to people aged 18 and over, in their own home, to help them avoid being admitted to hospital. It also supports people to return home following a hospital stay.

The service runs every day of the year from 8am to 8pm and teams are based across Sussex in:
  • East Sussex - Brighton and Hove, and Lewes
  • West Sussex - Chichester, Crawley, Horsham, Haywards Heath and Worthing

The teams respond to referrals within two hours, providing rapid access to a range of health and social care professionals, including Health and Therapy Assistants, Occupational Therapists, Nurses, Paramedics and Physiotherapists.

Their short-term support helps people manage a health crisis or illness, medication needs, feeding and hydration, to ensure that they can remain in their own home safely and live more independently for longer.

Main duties of the job

As a Nurse, you will be delivering holistic assessments, planning, implementing, and evaluating treatment care plans, alongside reviewing care provision.You will provide input for patients including identifying and managing deterioration in the community by utilising a range of core skills. You will be part of a team supporting evidence-based care, facilitating staff learning and development.

This post is an ideal opportunity for a nurse who is keen to develop both their holistic assessment skills and knowledge of urgent community response. If you are new to Sussex Community NHS Foundation Trust, you will join our preceptorship programme, receiving protected time and regular clinical supervision with dedicated registered professional support.

Job responsibilities

To be successful in this role, you will need:
  • Professional registration with the NMC
  • A mentoring qualification or working towards this
  • Evidence of a range of clinical competencies appropriate for the role
  • Experience in a variety of clinical settings
  • Knowledge of Clinical Governance, clinical audit and clinical supervision, Professional codes of Conduct
  • Demonstrable competency in a range of clinical skills
  • Knowledge of current local policies informing community health and social care services
  • A clear understanding of the role of the Nurse working in the community
  • To beable to demonstrate the application of research based practice
  • To beable to promote independence and choice in undertaking patient care
  • IT literate or willing to achieve within an agreed period
  • To be able to demonstrate knowledge/understanding of boundaries of role
  • Good numeracy and literacy skills
  • Good communication and negotiation skills
  • To be a car driver with a valid driving license for use in the UK and access to a vehicle for work purposes
  • To be willing to undertake training to develop within the role
