An exciting opportunity has arisen for a highly motivated Band 6 Occupational Therapist with an interest in providing community based intensive rehabilitation to people in their own homes within the NELFT's Intensive Rehabilitation Service (IRS). The IRS is a large multi-disciplinary consisting of occupational therapists, physiotherapists and nursing staff.

We are an ever expanding community based service with lots of new and exciting service developments happening right now. These include discharge to assess, in reaching and early supported discharges and we are looking for a motivated and enthusiastic band 6 to help guide these!

We provide a service from 8am - 8pm over 7 days. Our working days cover a two shift pattern - an early shift (8.30am - 16.30) and a late shift (12noon - 8pm). All staff generally complete 2 late shifts per 4 week roster bar. To provide cover for weekend shifts, OTs generally complete 1 day out of the weekend every 4 weeks. Shifts completed are flexible within the team however can vary depending on service demand.

Within IRS we offer regular supervisions, informal and formal, monthly OT CPD meetings both within IRS and an opportunity to attend trust wide CPD sessions. We have a comprehensive in-service training timetable and access to external relevant training opportunities.

The post holder will achieve this by working as part of the Intensive Rehabilitation Service (IRS) to provide high quality, timely rehabilitation for patients requiring step up from the integrated health team or step down from an acute or community bed.

Delivering high quality rehabilitation in a patient's own homes rather than in a community bed setting.

Providing high quality, patient-centred, goal orientated rehabilitation, demonstrating independence and self-management throughout the continuum of care.

Completing timely onward referrals to other community services including health and social care to ensure the safe management of the patient when completing their IRS episode of care.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High-Cost Area Supplement - London

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of �4,313 and a maximum payment of �5,436).

  • Priortise workload, supervise junior staff, work flexibly, data collection and audit


Clinical skills / Experience

Essential

  • Completed junior rotations
  • Previous experience in clinical governance and clinical audit and the implications to the service
  • 18 months post registration experience as an Occupational Therapist in relevant clinical settings
  • Junior Rotations - Neurology, medical, community, mental health etc


Desirable

  • Previous community rehabilitation experience


Knowledge

Essential

  • An awareness of NHS Plan, NSF and clinical governance priorities
  • Good knowledge of current best practice in Occupational Therapy in acute, physical health, community and rehab settings.
  • Previous experience in clinical governance and clinical audit and the implications to the service
  • Knowledge of current NHS and Primary Care agenda
  • In depth knowledge of national policies and procedures relevant to safe guarding vulnerable adults
  • Good sound clinical knowledge


Skills

Essential

  • Basic awareness of IT and IT skills

  • Demonstrate ability to work as an effective team member with excellent interpersonal skills
