We have an exciting opportunity for a motivated individual with a demonstrable interest in working with patients who are living with a mental illness in the community.

We are seeking a dynamic and enthusiastic Individual who is committed to working in partnership with patients/carers and a range of other care staff and agencies to provide truly person centred services.

You must have excellent communication skills, commitment to working flexibly to meet patient's needs, be able to demonstrate that you can work effectively in a multi disciplinary team setting and demonstrate a commitment to further development with the support of the team. You must have an awareness of current health and social care issues that affect community care for Adult with mental health needs. You will be expected to take an active role in facilitating the depot and Clozaril clinic's as well as the physical health monitoring for the team.

You will have commitment to your own development including experience of a varied caseload, health promotion and clinical supervision. Your active participation in providing a variety of learning opportunities for students and participating in training initiatives in general will be a part of the role.

The post also offers the opportunity to actively influence and participate in the development of services to Adult with mental health needs living in the South West area.

Main duties of the job

The post holder will support the Physical Wellbeing Nurse for the Recovery & Wellbeing Team. The post holder will be responsible for providing high quality, effective care and management of the depot clinic, clozapine clinic and physical health checks. They will be a member of a multi-disciplinary team and work in close co-operation with hospital based and primary health care based professionals. They will be responsible for the maintenance of standards of care within the Recovery & Wellbeing Team and actively participate in the training and education of students and staff seconded to the service. In addition, they will also be responsible for ensuring that an integrated assessment and treatment service is provided to the clients and ensure that a good inter-disciplinary working environment is maintained.

Job description

Job responsibilities

To establish and maintain effective individualised care using the Care Programme Approach (CPA) by:-

a) Undertaking assessment of risk and formulating care plans to manage and minimise risk. Planning, implementing and reviewing interventions designed to meet assessed needs.

b) To facilitate the depot clinic, requiring staff to travel to various locations within the locality, undertake physical health checks and taking bloods, support the clozapine clinic on site

c) Providing advice and information to service users, their carers and their families in order to alleviate distress and work in partnership, as far as possible, with all the available support systems.

d) Providing clear, accurate and concise documentation of the service users care as required by the Trusts Care Programme Approach (CPA) Policy.
  • To participate in team meetings, initiating and discussing new methods/models of care and participate in the introduction of research or trails within the area.

  • Seek to establish a therapeutic relationship with individual clients and their families.

    Ensure effective and confidential communication between all care professionals.

    Provide significant data governing client care and treatment by:-

    Participating in informal or formal case conferences and discussions.

    Maintaining a secure system of record keeping and information exchange

    Establishing regular written reviews of progress

    Ensure that information is recorded on the client information database.

    To develop close links with relatives/carers and ensure that effective communication takes place especially when clients are admitted or being prepared for discharge from hospital and that all appropriate agencies are informed, so that follow up care can take place.

    To advise the manager and Professional Nursing Advisor on all breaches of nursing professional conduct as required by the Nursing and Midwifery Councils Code of Professional Conduct.

    To liaise with the Manager to ensure that all (Eput) Essex Partnership University Trust policies and procedures are communicated, implemented and developed within the area.

    To participate in professional supervision on all allocated cases.

    To take on clinical work commensurate with experience and skills, in discussion with supervisor, and senior staff.

    To liaise and work in partnership with the voluntary sector, maintaining a close relationship for the benefit of our mutual client group.

    Out of hours and weekend working may be required according to the needs of the service,

    Person Specification

    RMN/ RGN oRMN qualified for at least 6months

    Essential

    • oknowledge of physical health monitoring / health promotion (2)
    • oCar driver, access to vehicle (2)
    • Experience and interest of working with people with Mental Health problems for at least two years


    Desirable

    • Venepuncture/ECG trained
    • Experience of facilitating Clozapine Clinic
    • Experience or willingness to undertake training in ECG and venepuncture (2)
