Reporting to the Senior Nurse/Operational Lead for CaVHIS, The Health Inclusion Nurse (HIN) works as part of the CaV Health Inclusion Service to improve the health of the most socially excluded patient groups in the Cardiff & Vale area. The HIN will take a positive, innovative and solution focused approach to providing timely care for people experiencing poor health outcomes who present with varying health complexities. The HIN will work closely with general medical colleagues, nurses from other specialities and tertiary agencies, to encompass the best achievable health outcomes. The HIN will develop and deliver intervention and act as an expert source of specialist advice to foster a culture of change towards health inequalities and injustices and advocate for accessible health care. The HIN will be dedicated, motivated, compassionate, adaptable and highly clinically skilled, they will have the professional confidence to work in an unpredictable and fast paced environment, they will have the ability to formulate high level holistic assessments considering all presenting concerns, create an excellent standard of risk assessment and identify any safeguarding concerns, referring to onwards general medical services when appropriate.

Main duties of the job
- Triage and treat those presenting to the general medical services/primary care with varying health complexities
- Refer and signpost to relevant care/specialities/agencies where necessary
- High level risk management, risk assessment and safeguarding skills
- Identifying presenting health difficulties/risks of socially excluded populations
- Knowledge of minor illness and chronic disease
- Ability to be professionally confident and have experience of being an autonomous practitioner and working within a team
- Ability to work with people who have experienced trauma
- Ability to engage with colleagues and other professionals to reduce poor holistic health outcomes and have previous experience of extended interagency working
- Experience of working with people where English is not their first language and using various means of interpretation
- Ability to engage in transitional change and service improvement
- A non-judgemental attitude and compassionate acceptance of others is crucial to our service.

- Use a thorough and structured approach to gather and document clinical history considering physiological, psychological, social and spiritual needs and any presenting safeguarding concerns including public health screening
