Working within the legislative framework of part One of the Mental Health (Wales)Measure, to improve access to mental health support at a primary care level for peoplewith mild to moderate or stable severe mental health needs within the context of a multidisciplinary multi-agency Primary Care Mental Health Support Service (PCMHSS).The service is delivered to working age adults, older adultsand people with a learning disability

Main duties of the job

An exciting opportunity has arisen for a dynamic and forward-thinking professional with experience of working with adults to join the Torfaen Primary Care Mental Health Support Service (PCMHSS).

Applicants must be a Registered Mental Health Professional (Occupational Therapist, Registered Nurse - Mental Health, Registered Nurse - Learning Disabilities or Social Worker).

The successful applicant will support the wellbeing of adults by delivering the five functions of Part One the Mental Health Measure:

o Conducting assessments of mental health needo Providing brief, evidence-based therapeutic interventionso Offering consultation to professionals working at a 'Tier One' levelo Providing guidance and advice to adults and their familieso Supporting onward referral where necessary.

Ensuring effective service provision to adults is a high priority within the Gwent PCMHSS.

There will be training and developmental opportunities for the successful applicants, as well as the opportunity to contribute to new initiatives and developments within the service.

This is an opportunity to work within a psychologically minded service with a strong commitment to the prudent healthcare agenda. Informal enquiry about the posts is welcomed.

Person Specification



  • professional qualification and valid registration


  • additional qualifications ie intervention and supervision or mentorship



  • Evidence of achievement in clinical practice within the area of speciality eg primary care, community mental health, older adults, CAMHS
  • Experience of delivering psychological interventions
  • Experience of managing own caseload and working independently


  • Experience of supervision/mentorship



  • Assessment skills including mh wales measure and safeguarding/risk management
  • Evidence based informed practice re Ax and intervention eg matched care & individual responsivity
