We are excited to be expanding our Advanced Practitioner (AP) workforce across (Medical Assessment Unit) MAU, Same Day Emergency Care (SDEC), Geriatric Emergency Medicine (GEMs), Older Persons Assessment Unit (OPAU) and our Emergency Department (ED) to support planned changes to Urgent Care at Weston General Hospital due to Healthy Weston 2.

Healthy Weston 2 outlines our Vision to support development of a service where patients are seen and treated in the right place and time and are discharged home without unnecessary delays. A great amount of this work will take place at the front door within our Urgent Care.

The programme aims to see Weston General Hospital as a centre of excellence for frailty and elective surgery within the wider UHBW Trust.

Here at Weston, there is an available apprenticeship route for AP as a pathway which will be supported by a senior AP and Consultant AP, in addition to the consultant body. Since the merging of our Trusts there is also great opportunity for collaborative working with Bristol and One Weston locality

Interested applicants for all posts must have significant experience at a senior level working within an urgent care setting and have experience to support the ability to see and treat patients autonomously within the desired environment. We require enthusiastic, forward thinking, well organised and motivated individuals with a positive attitude to join our multi-professional teams to work within Urgent Care at Weston General Hospital

Main duties of the job

The Advanced Practitioner (AP) will use specialist knowledge and skills to providehealthcare independently to patients. The AP is accountable for the total episode of care for patients with undifferentiated and undiagnosed needs.

The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness in care.Theywill be requiredto work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients.

The trainee AP will participate in delivering the care described above whilst working towards gaining their NHS England Digital Badge or equivalent accreditation from relevant college (e.g. RCEM).

Knowledge and Experience

Essential

  • Significant post registration experience with evidence of autonomous practice
  • Experience in acute presentations
  • Experience in care of the older person and complex needs
  • Evidence of supervision/mentoring/teaching
  • Ability to organise and prioritise own workload effectively
  • Familiar with current local and national issues
  • Evidence of leading self/teams


Desirable

  • Experience or sound knowledge of change management as part of a leadership role
  • Experience of developing advanced practice guidelines, governance and audit


Qualifications and Training

Essential

  • Current professional registration with NMC/HCPC/GCP/GPhC
  • Evidence of formal ongoing professional development
  • Evidence of maters level study, eg working towards a Masters in Advanced Clinical Practice
  • Teaching Qualification or equivalent experience


Desirable

  • MSC in Advanced Clinical Practice
  • Physical Assessment Module
  • Independent Prescribing

  • Possess the skills and wide range of clinical knowledge to work as an autonomous practitioner to undertake independent, direct consultation and provision of care
