Take a look at our Palliative Care Lead (development) band 6 post as part of the Palliative Care Coordination Centre team but based at Norton Canes.

This post aims to equip the post holder with the necessary skills, knowledge and experience to become future leaders in palliative care.

You must be a first level registered nurse with post registration experience of palliative/end of life care and have evidence of relevant continuous professional development.

The services operates 7 days a week 365 days a year with core contracted hours of operation between 9am and 6pm.

This post is based at Bradwell Hospital in Newcastle under Lyme.

Main duties of the job

As a member of the Palliative and End of Life Care Team the post holder will work under the direct or indirect supervision of the Palliative Care Leads and be accountable to the Nurse Consultants. As a band 6 professional you will be expected to work in line with your achieved competence framework with minimal direct supervision on a day to day basis.

They will support the development and implementation of Palliative and End of Life care work programmes and initiatives across MPFT.

The post holder will support with a range of education and training to a diverse workforce, working in partnership with a number of health and social care professionals, providers and agencies.

The post holder will assist in development and implementation of appropriate pathways of care for people with Palliative and End of Life Care conditions.

This developmental role is designed to equip the clinician with the skills, knowledge and experience through a structured development programme. This will be designed around the role progression requirements and the individuals existing skill knowledge and experience. This may take up to approximately 2 years (but not guaranteed) dependent on individual progress towards agreed goals and the availability of vacant band 7 posts. The development role is designed to prepare staff so that they can become future leaders in palliative care and be fully prepared to apply for a Palliative Care Lead post as these posts become available.

Job responsibilities

Main Duties and Responsibilities

To act as an advisor in palliative and end of life care, maximizing the potential of community services and enhancing the efficiency and effectiveness of discharge pathways and processes to meet the modern NHS agenda, through developing the knowledge, skills and competence of team members.
  • Work in partnership with a range of medical staff, community nursing, allied healthcare professionals, specialist palliative care providers, social care and other agencies in the implementation of patient care to facilitate integrated, effective working between the MPFT workforce and the wider health/social care team.
  • To participate in complex skilled nursing activities directly related to the assessment, planning and evaluation of patient care. including those with complex medical/nursing needs, in order to support and empower MPFT staff, under the direct or indirect supervision of the Palliative Care Leads and be accountable to the Nurse Consultants.
  • Collaborate with Palliative Care Leads / senior clinical colleagues and medical practitioners to lead the implementation of care and treatment plans reflecting patient / carer needs and preferences for Palliative and End of Life Care.
  • Utilise knowledge of medicines management and decision making skills in relation to this.
  • Develop clinically effective and therapeutic working relationships with patients and relatives using advanced communication skills, to enable patient and family to make informed decisions, participate in care planning and discuss complex and contentious sensitive information related to end of life issues.
  • To build relationships across professional and organisational boundaries across the health economy, breaking down barriers and enhancing the patient experience.
  • Support nursing practice within palliative care and understand opportunities to influence local strategy.
  • Role model, teach and support teams to utilise validated tools to promote provision of high quality palliative and end of life care services.
  • To support programs of clinical audit consistent with MPFTs governance framework and national palliative and end of life care quality standards.
  • To support and contribute to multidisciplinary palliative care clinical meetings across a variety of care settings.
  • To participate in peer support, education and clinical mentorship with other members of the palliative and end of life care team. To participate in regular supervision during the development role and give verbal reports to the Nurse Consultants and undertake regular evaluation of the role. To work with the Team and identify individual needs to allow the development of the clinician into a palliative leadership role.
  • To maintain an up to date level of research and evidence based professional knowledge and competence.
  • Support the development of the service by ensuring patient and public engagement.
  • Participate in service development through undertaking need assessments and mapping activities to identify service gaps and service development opportunities.
  • To ensure that the service is safe, caring, responsive, effective and well led through promoting best practice, investigating and learning from incidents, utilising research and audit findings.
  • To support with the delivery and evaluation of a range of education programs for students and multidisciplinary colleagues consistent with the workforce training needs analysis and education strategy.
  • To support the development of competency frameworks and education packages to underpin standards and policies to achieve a consistent approach to palliative and end of life care across MPFT.
  • Contribute to the delivery of a Trust wide multi-disciplinary, palliative and end of life service.
  • To support and participate in the training and development of staff, and where appropriate colleagues from the voluntary and independent sector, as well as students, carers and patients.
  • To demonstrate and ensure good practice of record keeping, both written and electronic.
  • To adhere to the NMC Standards of Conduct, performance and ethics for nurses.
  • To promote personal, professional, and clinical expertise through regular supervision, evaluation and continual professional development.
  • To participate in appraisal and personal development planning with line manager.
  • To support a range of colleagues with the identification of patients with palliative or end of life care needs, irrespective of diagnosis or location and utilising recognised tools and metrics to inform practice and decision making.
  • To provide a strong supportive role to the Palliative Care Centre during its hours of operation.

Person Specification



  • RGN/RN Degree level qualification
  • Palliative Care course OR relevant significant palliative care experience as an RGN/RN.
  • Evidence of robust CPD relevant to specialism.


  • Leadership Qualification/study
  • Independent prescribing
  • ENB 998 / Mentorship course



  • Experience of mentoring pre-registration staff
  • Post registration experience where a degree of autonomous practice has been required
  • Recent significant experience of providing direct clinical care to palliative care patients.


  • Delivery of teaching programmes and presentations to a range of audiences
  • Primary/community care experience
  • Experience of working with wider primary care team, and across agencies e.g. voluntary / independent sectors, social care.
