Does a role supporting older adults with mental health problems motivate you? Do you have a passion for enabling older people to maintain the relationships that are important to them?Perhaps you have a keen interest in working with people with dementia?If so we would love to speak to you about our Dementia Assessment Nurse role in our Specialist Assessment Service, covering the beautiful coastal West Sussex region. Covering Chichester, Bognor Regis and surrounding rural localities, we work in partnership with the Alzheimer's Society to provide diagnostic assessment and post-diagnostic interventions forpeople with suspected dementia.Our work is focussed on assessment of potential dementia, so although we support a large number of people, we don't carry caseloads for long periods of time.
Main duties of the job
Our main aim is to support people thorough the assessment process and in accessing the right treatment and support for their individual need(s).We use a standardized assessment process in conjunction with the whole multi-disciplinary team to help resolve concerns around cognition and potential dementia for our patients and their families. Once a diagnosis has been formulated we then provide the appropriate support, signposting and advice to our patients and their families to help them live well with theirdiagnosis.Our service is here to support patients and carers with concerns about their cognition and the provision of post diagnosis follow up if it transpires that they have dementia. Early diagnosis is a fundamental component of the National Dementia Strategy and aims to empower people and their carers to make decisions about their future based on early diagnostic information relating to their condition.

See attached Job Description and Person Specification for full details.Requirements:In order to be a successful Community Psychiatric Nurse in our team you will be:- A Registered Nurse with the NMC Mental Health, or LD- Passionate about supporting adults with Dementia or suspected Dementia- Experienced in conducting clinical assessments- Comfortable working in an autonomous role, managing your own diary/ caseloadIt would also be advantageous if you are:- Experienced working with older adults presenting with dementia or complex mental health difficultiesWe have ample opportunities for additional CPD, including specialist dementia training with The Royal College of Psychiatry.The service operates 0900 - 1700 hours Monday to Friday, with extra bank hours available for those who seek it.The ability to travel across the region is essential for this role, as you will need to visit people in the local community. SPFT staff have the capacity to work from home remotely where applicable and in agreement with your line manager.

- Completion of Mentorship Course/ENB equivalent

- Experience of working alongside people with complex health problems in the designated care group
