The post holder will provide evidence based palliative care, advice and support to patients with a diagnosis of cancer or other life limiting illness, and their carers. They will be accountable for their own professional actions without direct supervision. The post holder will deliver high quality palliative care (over a 7 day service) in partnership with nursing, medical and AHP colleagues across departmental and organisational boundaries. They will support palliative care education on a range of clinical and ethical issues across professional disciplines. The post holder will support clinical audits under supervision in their own area of practice and contribute to the process of Clinical Governance for the Trust.
Main duties of the job
As a Community Palliative Care Nurse you will be expected to manage a caseload of patients and their families/carers with palliative care needs, assessing, developing and implementing programmes of care.
You will provide palliative care advice and support to patients, carers and all health care professionals using specialist knowledge to address issues around palliative care including treatment, pain and symptom management, psycho-social needs and ethical dilemmas liaising with other Palliative Care Nurses as appropriate.
You will be expected to establish and maintain highly effective communication with patients, carers, and professional across health and social care in order to develop therapeutic relationships within which sensitive, distressing and complex issues at times will require addressing.
This role will involve you working asan advocate for the patient/family and / or carer when necessary.
