The Community Health and Integrated Care Division at East Sussex Healthcare Trust have a number of exciting opportunities.
Job Overview:
This is a 6 monthly rotational post within the Community Nursing Teams and Urgent Community Response Teams. You will support the Urgent Community Response and Community Nursing teams in the delivery of an efficient and effective nursing service and will have responsibility for assessment, planning, delivery and evaluation of care. We havebusy and varied caseloads, and you will be visiting housebound patients in their own home. You will be well supported in Practice to enable you to develop a large range of clinical knowledge and skills. Good communication skills are essential, along with the ability to work alone as well as part of a team.
Our Community Nursing Team support planned care and nursing intervention for patients in their homes.
Our Urgent Community Response Team serve both our community and acute patients by supporting early discharges onto a virtual ward and managing 2 hour urgent response calls.
Main duties of the job
Assessing new patients for nursing interventions in their own homes, creating & evaluating care plans, achieving optimal independence using best practice & following national & Trust policies & guidelines
Clinical skills include End of life symptom control, palliative care, IV administration, Diabetes management, Catheter care, Drain management, Wound care and leg ulcer management.

During your Community Nursing rotation, you will have the opportunity to work in one of our 8 Primary Care Network (PCN) Teams covering Bexhill, Hastings, Rural Rother, Eastbourne, Seaford and Hailsham. We provide a seven day service to patients from 08.00-20.00 hours and offer a range of flexible working hours.
During your Urgent Community Response rotation, you will have the opportunity to work in either Hastings and Rother Team or The Eastbourne, Hailsham and Seaford Team both based at ESHT Acute Hospital sites. This is a seven day service from 08.00-22.00.
