This is an exciting opportunity to work alongside a dynamic and highly respected team to support the rapidly expanding Home Oxygen Assessment and Review Service (HOSAR) and Pulmonary Rehab Services at Harefield Hospital. These services work closely with the Hillingdon Integrated Respiratory Services (HIRS) based at Hillingdon Hospital to provide comprehensive community respiratory services for patients living in the borough of Hillingdon.
The postholder will have the opportunity to work across both organisations and within the community to ensure continuity of the patient pathway, as well as championing pulmonary rehabilitation and responsible oxygen prescribing. There will also be opportunities to gain further experience in admission avoidance / early supported discharge pathways, virtual wards, home ventilation, specialist airways clinics and spirometry.
Main duties of the job
To provide advanced assessment and appropriate prescription of oxygen to cardio-respiratory patients by performing long-term and ambulatory oxygen assessments for patients on site and in the community, and to develop this service along with other members of the multidisciplinary team and senior physiotherapists.
To support the work and collaboration between Harefield Respiratory services and North-West London ICS in providing integrated respiratory care in the community. This may include providing community spirometry, patient education and community COPD clinics.

Job summary
Undertake a highly specialist team role in planning, co-ordinating, delivering and evaluating the Harefield Home Oxygen and review service and the Pulmonary Rehabilitation Service.
To take a lead role in the advanced assessment and treatment of patients within the speciality who may have highly complex needs, and to determine clinical diagnosis, nursing or physiotherapy treatment indicated, and maintain records as an autonomous practitioner. This could be incorporated into a multidisciplinary specialist COPD clinic.
To provide advanced assessment and appropriate prescription of oxygen to cardio-respiratory patients by performing long-term and ambulatory oxygen assessments for patients on site and in the community, and to develop this service along with other members of the multidisciplinary team and senior physiotherapists.
To assist with delivery of the pulmonary rehabilitation service within the hospital and community sites as required, and to coordinate development of the service, along with other members of the multidisciplinary team and the senior physiotherapists.
To contribute to the strategic direction of the home oxygen and review service as well as the pulmonary rehabilitation service, including the potential expansion of these services.
To support the work and collaboration between Harefield Respiratory services and North-West London ICS in providing integrated respiratory care in the community. This may include providing community spirometry, patient education and community COPD clinics.
To work alongside members of the Respiratory NIHR Biomedical Research Unit to continuously develop the service through research and audit, maintaining quality and direction.
