We are looking for a band 6 nurse with a specialist interest in integration, hospital discharge and flow. This is an exciting time to join the integrated discharge team, we are proactively engaged in a range of transformational projects, working towards the Trusts Vision of providing care closer to home and being rooted in our communities.
This role will evolve as we strive towards our vision of integrating services that you could be part of. The Discharge Team will provide you with the opportunity to work within an inter-disciplinary team, assessing and coordinating the discharge of patients with complex health needs . The team work collaboratively with Health and Social Care to ensure a patient' needs are met within the most appropriate community environment.
Main duties of the job
You will be responsible for coordinating the discharge of patients with complex health needs, implementing the agreed discharge model and collating information regarding the patients ongoing needs.
You will educate nursing and medical staff on wards regarding discharge pathways, the agreed timeframes, process etc, attending board rounds, MDT meetings and case conferences as required.
You will work collaboratively with both internal and external teams to facilitate safe, timely discharges, highlighting opportunities for future clinical pathways, and recommending potential improvements to discharge systems and processes.
You will participate in training, teaching, appraising and supervising of junior staff.

Job responsibilities
Reduce the length of stay and excess bed delays by the implementation of an agreed discharge model and to understand and collate information regarding the ongoing needs of patients no longer requiring medical care in an acute hospital setting. To assess complex Health patients and patients requiring fast track support, working with the patient and family to facilitate a safe, timely discharges. To lead the implementation of the agreed discharge model for patients within UHCW, working collaboratively with local integrated community teams. To assess and monitor progress through clinical pathways and agree solutions in dealing with deviations from the discharge pathway and timeframes. To educate nursing and medical staff on wards regarding the agreed timeframes, process etc, attending board rounds, MDT meetings and case conferences as required. To participate in training, teaching, appraising and supervising of junior staff. To record and review information on identified visibility tool in relation to the number of patients no longer requiring an acute medical bed and the reasons for their continued stay within UHCW. To work in partnership with acute wards to support the development and the local community teams to support the development and implementation of evidence based clinical pathways. To liaise with other agencies to ensure effective discharges.
