We need an experienced, enthusiastic and motivated individual to join our team as a Band 7 Diabetes Specialist Dietitian.
To provide a specialist dietetic service to patients with diabetes and their families under the care of the Consultant Diabetologists at Darent Valley Hospital
To act as a source of expertise in the dietetic care of people with diabetes to patients and their families, medical staff, other health professionals, General Practitioners and the Dietetic Department
To be the professional lead for the dietary management of patients with diabetes for the Diabetes Centre and Nutrition and Dietetic Department and for the Acute and Community Healthcare professionals
To be an integral part of the Multi-disciplinary Diabetes Team, working with professional colleagues
To provide education and training to medical staff, General Practitioners, Nursing staff, Student Nurses, Dietetic Students and other Health Care Professionals within the Acute and community setting
To provide structured education for people with T1 and T2 diabetes (DAFNE and DESMOND)
Main duties of the job
To provide a specialist dietetic service to all out-patients with type 1 or type 2 diabetes referred by the Consultant Diabetologists and local General Practitioners. This will include patients requiring a high level of care to minimize the progression of complications due to their diabetes and who have difficulties achieving optimal control.
To undertake comprehensive diet and lifestyle assessments and to formulate individualised patient management and treatment plans for patients with Type 1 and Type 2 diabetes. This includes ability to interpret patient's blood glucose record keeping, including continuous glucose monitoring (CGM) data and reports
To assess and educate individual patients in self-management principles e.g. carbohydrate counting, effective blood glucose diary monitoring and interpretation, constructive interpretation of continuous glucose monitoring (CGM) data and reports, application of related self-management approaches based on structured education principles in Type 1 diabetes patients developing carbohydrate awareness skills in Type 2 diabetes patients
To be a nominated 'gatekeeper' together with a Consultant Diabetologist and a specialist nurse, for patients transferring to insulin pump therapy.
