Newmarket Integrated Neighbourhood Team

Fixed Term or Secondment - 18 months

Join our dynamic Integrated Neighbourhood Team in Newmarket, where you'll play a pivotal role in shaping the future of health and social care across Suffolk. This key position focuses on managing professional staff within your area of expertise and significantly contributing to the leadership of our Integrated Neighbourhood Teams. Your role is crucial in integrating health and social care, collaborating with various partners to ensure seamless, accessible services for our community.

In this role, you will lead Community Health care professionals, striving for excellence in service delivery and rehabilitation for the people of Suffolk. As a champion of the Alliance's vision, objectives, and priorities, you'll inspire your team and partners, embodying our values and advocating a culture of cost-effectiveness, patient-focused outcomes, and ongoing improvement. You'll manage a group of professionals within Community Health, adapting to our evolving operating model and locality-based teams.

Working closely with Integrated Locality Leads, Adult social care partners, Operations and Partnership Managers, Alliance Delivery Leads, and key partners, you'll play a strategic role in the West Suffolk Alliance. Main duties of the job

JOB PURPOSE:

This is a key role within the organisation, located within the Integrated Neighbourhood Team. The main purpose of this role is to directly manage the professional staff within their own professional area in one of the Integrated Neighbourhood Teams, and contribute to the effective leadership of the Integrated Neighbourhood Teams, supporting the relevant Integrated Locality Lead in delivering good quality services and effective integration between health and social care through collaboration with stakeholders and partners across the system to deliver better outcomes for the people of Suffolk. Managing alongside key partners, system leadership will be a key part of the role, ensuring health and social care systems are joined up and accessible for all our service users.

You will lead and manage the community health / social care members of the Integrated Neighbourhood Team to deliver the highest standards of health, social care and reablement for the people of Suffolk.

There will also be Teams working in the locality i.e. Hospital to Home Team, Flexible Care and Support Team (FCST) where the Manager leads a team that works in an integrated way across the locality, supporting partnership working and development.

You will be expected to promote the Alliance vision, objectives, and priorities effectively to your staff, our partners, and the public and inspire others by role modelling organisational values at all times. You will promote a culture of value for money, outcome based customer focus and continuous improvement.

You will manage a group of professionals aligned to either Community Health or Adult Community Services within an Integrated Team in a particular geographical area of West Suffolk, as we progress to the emerging new operating model and locality based delivery teams.

You will work as part of the West Suffolk Alliance, working closely with your Integrated Locality Leads, Operations and Partnership Manager and Alliance Delivery Leads, Integrated Director, CCG transformation team and the mental health, primary care links and other key partners for the locality.

Experience & Knowledge

Essential

  • Good working knowledge and extensive experience of working within an Integrated Health and Social Care system
  • Good knowledge and understanding of the major issues facing local government, and the specific challenges facing Health and Adult Social Care.
  • Good understanding of all relevant health and social care specific legislation including the practical application of relevant legislation and practice guidance.
  • Experience of the management and supervision of staff
  • Documented evidence of continued personal development, relevant to community services
  • High level of ability to reflect on, and critically appraise, own practice and performance
  • High level of presentation and training skills
  • Experience of Microsoft Word and use of email Ability to manage and monitor delegated team budget
  • Ability to monitor and analyse data to assist in developing and managing system delivery from team perspective
