This is an exciting opportunity for a registered nurse to join our Older Persons assessment Service (OPAS) nursing team at Morriston Hospital. We are a specialist service that is embedded within the Emergency Department in Morriston Hospital at Swansea Bay university health board. You will work as part of a multi-disciplinary team, in the delivery of individualised care to the older person. You will be part of the team that will complete a comprehensive geriatric assessment, a personalised management plan for each patient individually who is over 65 and has a frailty syndrome, polypharmacy, injury secondary to a fall, care dependence, poor mobility and function, confusion, delirium or dementia.

We are a team that includes staff nurses, health care support workers, an emergency nurse practitioner, advanced clinical practitioners, Occupational therapists, Physiotherapists, a physician associate and Geriatric Medical Consultants.

We are a developing service that is expanding and changing daily and is the first service in the UK to have an emergency nurse practitioner (ENP) within its team. We accept patients from several entries and have an ENP outreach service that will prevent patients with specialist needs to attend the hospital. This will bring many opportunities for a staff nurse to be part of the OPAS journey and possibly develop their skills into the next future emergency nurse practitioner, advanced clinical role or a geriatric clinical nurse specialist.

The post holder is responsible for the assessment, planning, implementation and evaluation of evidenced-based nursing care, working collaboratively and cooperativelywith others to meet the needs of patients and their families.

Assist in the management and organisation of their clinical area and participating in the education, development and supervision of other staff members.
