This is fantastic opportunity for a compassionate Mental Health Practitioner to join a service that provides health care to the homeless population of Derby City, liaising with both statutory and voluntary agencies to ensure best outcomes for the service user.
You will be working as part of the community mental health team, focussing on those who are homeless.
Your patients may be vulnerable individuals with complex health and social care needs, such as drug and alcohol dependency and mental health problems.
We are looking for a non-judgmental, skilled and dynamic individual with a commitment to providing high standards of care for this vulnerable group.
You will need to have excellent communication skills, be an excellent problem solver and have the ability to work alone.

Currently we are undergoing a transformational change in how we deliver mental health services to people living in Derbyshire.
In February 2020, NHS England published a new Community Mental Health Framework (CMHF), which is a 3 year NHS programme aiming to improve care for people with Severe Mental Illness (SMI).
In Derbyshire, this new way to offer holistic health and wellbeing support in the community will be called Living Well Derbyshire.
This model will offer easily accessible support from a range of organisations in one place - social care, housing support, physical health support - all of which can have an impact on a persons mental health and wellbeing.
The new way of working will aim to reduce barriers to service and by working better together we can aid in recovery and long term wellbeing for the future.
A wide range of professionals dealing with a wide range of issues to reduce unnecessary referrals and handover but with one point of contact and with an easy way back into services.

- Manage a caseload delegating and referring to nursing/social work/OT and GP colleagues, as well as external services as appropriate.
- Act as the patient's advocate and, by providing information and support, facilitate the patient's own choices with regard to nursing care, promoting independence and self-care as appropriate.
- Adopt a proactive, flexible and opportunistic approach in order to promote continuity of care.
- Undertake and monitor care, using valid evidence-based practice that maintains or improves the health and well-being of the target population, referring as necessary to other services in accordance with established protocols.
- Undertake a comprehensive range of activities and have key delegated responsibilities for defined areas of care
- Autonomously assess and treat patients presenting with significant mental health issues.
- Where appropriate responsible for gaining valid informed consent for relevant assessment and associated intervention processes.
