We are looking for an enthusiastic and highly motivated nurse with critical care/ acute clinical and leadership experience to join our Critical Care Outreach team (CCOT).

The role of the service is to deliver a critical care outreach service to adult inpatient areas at The Whittington Hospital in North London.

The successful applicant will join a supportive and dynamic team, leading on delivering high standards of care to acutely unwell patients across our site. CCOT is a nurse led service supported by the critical care consultant team. You will have the opportunity to work both as a team and independently, identifying patients at risk of clinical deterioration or who are actively deteriorating. The CCOT nurse then needs to be able to carry out appropriate assessment, interventions and ensure adequate on-ongoing management.

Teaching and delivering education to ward teams is an essential part of the role together with leading on deteriorating patient quality improvement work across site.

There will be opportunities and expectations for you to further your professional development with access to courses available through the Trust and with our partner education institutions.

Working pattern is to cover a rota of 12-hour day shifts (08:00-20:30) 365 days a year.

Job responsibilities

  • To be a clinical expert and role model in the field of critical care nursing and practise within the NMC code of professional conduct.
  • To have a highly visible clinical presence and the ability to work autonomously.
  • Support clinical staff with clinical decision-making, therefore aiding their professional development.
  • Assessment and management of critically ill patients throughout the Trust.
  • Respond to referrals from adult inpatient areas. Review and respond to NEWS (Early Warning scoring system) as used on the trusts vitals system.
  • Assist all staff in identifying patients who are at risk of deterioration, ensuring quality of patient care improves through early identification of these patients.
  • To undertake and complete within 1 year of joining the team, competencies as required by the Trust.
  • To utilise advanced skills in patient assessment and to undertake appropriate clinical decisions and actions without supervision and refer patients to medical personnel as appropriate.
  • To provide clinical advice and expertise where necessary in relation to patients who have recently been discharged from intensive care or who have the potential to deteriorate, with an emphasis on supporting, teaching, and empowering ward staff.
  • The post holder will work collaboratively with the MDT, making and accepting referrals, prioritising and acting in a timely manner to rapidly optimise treatment and prevent further deterioration of the acutely ill patient.
  • The post-holder will be required to routinely perform, order, and interpret a variety of tests, investigations and procedures required to make a comprehensive diagnosis and deliver treatment, such actions involve significant concentration and highly developed physical skills. These include a wide range of blood tests, radiological investigations, arterial blood gases, venepuncture and cannulation.
  • To be competent in basic airway management, working closely with ward staff delivering CPAP, HIGH FLOW and NIV. To have specific responsibility for supporting ward staff during and after any emergency event.
  • Working collaboratively with MDT in decision-making and the planning of care for patients with a tracheostomy or laryngectomy.
  • To work in collaboratively with the Pain Team, SALT, Physiotherapy and Nutrition team.
  • To be able to safely transfer a critically ill patient within the hospital as the need arises.
  • Appropriate management of bereavement including supporting Do Not Resuscitate and treatment escalation planning discussions.
