The permanent posts are eligible for a �1500 New Hire Bonus. An initial payment of �500 will be paid in the first 1 month and a final payment of �1000 will be paid 1 year after your start date. We are currently looking to recruit a full time enthusiastic and motivated Band 6 Mental Health Practitioner (Mental Health Nurse, Occupational Therapist or Social Worker) to join the Fenland Neighbourhood Team. This is a great opportunity to join our friendly, dynamic and busy adult Neighbourhood Team.

Based at Doddington Hospital,you would be a part of an established, well-integrated team including consultant psychiatrists, CPN's, psychologists, peer support worker's, social workers and OT's with links to other specialist services within CPFT and a very interesting and developing 3rd sector.

Main duties of the job

As a practitioner in the team you will act as a Care Coordinator for people who use the service, conduct physical health checks and promote Carer involvement in the delivery of care with service user consent.

You will have the opportunity to manage a caseload of service users, delivering a diverse range of clinical skills and building a therapeutic and recovery based relationship. We seek to empower service users to actively engage in their recovery journey, working toward an effective discharge with ongoing support from a wide range of third sector organisations and support.

The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under pressure at times, and with a desire for innovation. We are able to support agile working for staff, using all appropriate I.T equipment and support.

This service covers Fenland and surrounding rural areas therefore, successful applicants will need to be able to travel independently.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. • Mental Health practitioners in Neighbourhood Teams will provide assessment and interventions for people with significant mental health conditions
• Offer training, advice and guidance to colleagues in the neighbourhood team on common mental health conditions.
• Participate in Neighbourhood Team MDT meetings and ensure key information is recorded in shared care plans
• Work closely with the mental health integrated care team in order to provide an excellent mental health service.

Person Specification

Qualifications

Essential

• Relevant professional registered qualification (Registered mental health nurse or Occupational therapist)
• Training in supervision and appraisal

Desirable

• Post qualifying training
• Non-medical prescribing qualification

Experience

Essential

• Significant experience in working within a mental health service.
• Experience of working with older people with mental health needs
• Registered Student mentor
• Experience of clinical and managerial supervision of junior staff

Desirable

• Experience of working with an older people's community mental health service.
• Have experience of mental health challenges

Skills & Abilities

Essential

• Able to demonstrate commitment to high quality care and service provision
• Excellent clinical skill in assessment and interventions for common mental health conditions
• Excellent communication skills and ability to summarise information and communicate with colleagues.
• Excellent understanding of the needs of carers
• Ability to communicate effectively with people in distress or with communication difficulties

Desirable

• Experience as safeguarding lead
• Experience of doing Best Interest Assessments

• Excellent understanding of statutory framework and care commissioning including: safeguarding, commissioning of care and Mental Health Act and Mental Capacity act
