This post is eligible for a Golden Hello payment of �2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays

Are you a motivated District Nurse looking for change? We are seeking our next District Nurse to join the Eastwood District Nurses Team.

The Adult Community Service is an integrated healthcare team providing long- term conditions management, rehabilitation and community nursing services within the Broxtowe area. We are an enthusiastic team with a breadth of experience and a shared goal to offer the highest standard of care to our patients. We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in both a home and clinic setting.

Main duties of the job

Your role will be to lead a team of staff and case manage the provision of care within an individual's home or residential home, including palliative care and long term conditions management. You will work autonomously using your own initiative to deliver holistic care to adults with physical health needs.

A key aim of our service is reduce unplanned hospital admissions and facilitate discharges from the acute care environment. The team members include physiotherapists, occupational therapists, Community nurses, community matrons, specialist nurses and support workers. We also work closely with GP's and the voluntary sector.

Job description

Job responsibilities

You will have a Specialist Practitioner qualification in District Nursing, a degree/ diploma in nursing and be registered with the NMC. You should possess excellent leadership abilities, interpersonal skills and time management skills. You should have experience in caseload management and be able to priorities work. An induction period will be provided.

A full UK driving licence and vehicle for business use is required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Person Specification



  • NMC Registered Nurse Part One and holds current and valid registration with the NMC
  • NMC recordable qualification in District Nursing inclusive of V100/V150 prescribing qualification at degree level or equivalent
  • Further specialist knowledge/post registration experience in specific service area or other formal training.


  • Demonstrates continued professional development to maintain professional registration
  • Holds a mentorship qualification and has ability to provide mentorship to others



  • Willingness to undertake new clinical practices, education and training that will enhance service delivery



  • Demonstrates satisfactory clinical expertise for the banding.
  • Specialist and area specific experience


  • The ability to deal with and resolve conflict



  • Knowledge and understanding of current legislation and Trust policy, values and objectives
  • Awareness of national and local issues impacting on nursing care
  • Ability to seek out current best practice based on research and evidence based enquiry
  • Knowledge of the nursing 6 c's
  • Understanding of clinical risk assessments and risk management



  • Ability to manage a caseload and lead and manage the nursing team within an integrated and interagency context
  • Good holistic and person centred assessment skills
  • Ability to plan, perform and evaluate complex patient's nursing care including self-care initiatives
  • Ability to promote health and wellbeing both at individual and population levels
  • Good written/oral and IT communication skills
  • Ability to be flexible and diplomatically handle conflict issues

  • Clinical supervision skills
  • Full driving licence and access to car for work purposes

  • Ability to demonstrate RCA skills in investigating reported incidents
