Main duties of the job

The post will lead the implementation of a variety of specialist Speech and Language Therapy assessments and interventions: leading on the Speech and Language Therapy contribution to: assessment, interventions, care planning and evaluation.

The overall purpose of the post is to ensure that individuals who are referred to Speech and Language Therapy have their swallowing and communication needs assessed.Provision of interventions and recommendations based on needs identifiedThe Speech and Language Therapist will incorporate needs into an overall formulation of need and appropriate plan of care.

The post holder will be available for consultation, support and clinical supervision to other health professionals working within the in- patient Service and other areas of the Trust where identified as appropriate.

Job responsibilities

Main duties and responsibilities (Clinical):Ensure the delivery of high quality Speech and Language Therapy assessment, interventions and evaluations across SSSFT Mental Health In patient services based at The Redwoods. The work is largely across older adult wards including cognitive impairment.Demonstrate high personal standards in clinical practice.To ensure interventions are provided in line with NICE Guidance, RCSLT guidelines including other relevant local and national documentation. Responsible for prioritising workload, allocation of work to oneself and other staff, and to cover all levels of priority of work within the service.The majority of the work is likely to be specific swallowing assessments, with somecommunication assessments and, as part of the Single Assessment Process, culminating in an agreed plan of action and or care and intervention. This would include the use of recognised/agreed standardised Speech and Language Therapy assessment tools alongside profession specific and generic outcome measures

To make clinical judgements backed up by clinical reasoning and recommendations regarding appropriate actions/sign posting in consultation with multi-disciplinary staff and in accordance with the service specification.See service users on the in- patient wards, to carry out specialist assessments (within service specification) to identify swallowing and communication needs, and if indicated develop an agreed action plan of care.Complete and contribute, where appropriate to robust risk assessments and risk management plans.Deliver a range of speech and language interventions following specific speech and language therapy assessments to enable understanding of any expression of an unmet need and to maximise an individuals abilities.Lead the management of a complex caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process).Attend clinical meetings as appropriate to facilitate a safe and timely discharge into a community setting.Knowledge of national, regional and local drivers underpinning service delivery including the provisions of the Mental Health Act 1983 (revised 2007) regulations including Mental Capacity Act (2005), RCSLT GuidelinesUp to date knowledge and experience of clinical developments in Adult Mental Health.Adhere to RCSLT , HCPC Professional Code of Practice , Standards andGuidelines, and understand how to raise concerns if poor practice is witnessed

Implement highly developed physical skills pertinent to the area of specialism, for example manual handling, computer and keyboard skills, health and safety, deescalation skills and drivingActively maintain and demonstrate an up-to-date knowledge of national guidelines, standards, legislation and professional developments in current methods in the treatment of clients conditions within the specialism.Education, Service Development, Research, administrative duties:Identify and highlight to the Ward Manager and AHP Lead relevant issues in relation to the speech and Language therapy for people and their carers, assessed using best practice evidence, in order that these are reflected in service planning, development and future provision.

Develop efficient and relevant health promotion activities to promote healtheducation and ensure that preventative approaches and interventions are used to maintain independenceAs required, supervise junior staff provide clinical advice/supervision support, leadership and professional supervision, in line with Trust Policies.

Participate in staff appraisal and performance reviews as identified.Provide a learning environment to support students (multidisciplinary) undergoing training and assist new and junior members of staff, peers and other disciplines to develop professional competence.Lead on the development and delivery of specialist Speech and Language Therapyeducation and training to: clients, carers, families staff, students and other relevant organisations.Actively engage in the organisations clinical governance initiatives, for example to participate and lead in audit, clinical supervision and bringing critically evaluated evidence and research into practice.Participate and lead on research within area of clinical expertise where possible to raise understanding and awareness of the benefits of Speech and Language Therapy and awareness of communication and swallowing difficulties.To attend regular meetings with the Team, as agreed in the Operational Policy.To adhere to the Royal College of Speech and Language Therapists Code of Ethics and HCPC Professional Code of Conduct, and Core Standards for Speech and Language Therapists.
