The post holder will be required to support developing, the inpatient and community provision for diabetes care as the initial focus areas. However, in time the opportunity to work with a range of other patient's groups. Including Ante-natal clinic at Tameside General Hospital, community clinics, specialised clinics (i.e. Libre sensors and insulin pump clinic and young persons' clinic) will be provided. The role also includes Triaging referrals into the service, Telephone consultation.

The post holder will also be expected to deliver on structured education programmes for patients and HCP's. We are looking for a self-motivated and dynamic individual with a minimum of 3 years' experience of managing Diabetes as a primary condition for the band 6 have a sound clinical knowledge of Diabetes management and related therapies. To have completed a level 5 course in the management of Diabetes or happy to work towards

The clinical specialist will work as an autonomous practitioner to provide a high quality, efficient and effective clinical service, delivering high standard of assessment, diagnosis, treatment and highly specialist expert advice to patients with Diabetes and their carer's in accordance with the levels of the competences as defined in the KSF. In addition, they must be able to communicate effectively, work well within a team environment. Have good organisation skills and the ability to prioritize workload are essential

Main duties of the job

  • Has continuing responsibility to ensure the assessment, implementation and evaluation of specialist packages of care, are carried out to meet the nursing needs of a defined caseload. This maybe inpatient or outpatient care.
  • To provide care to people with Diabetes and other associated disorders, some of who may be seriously/terminally ill and occasionally present with aggressive/challenging behaviours.
  • To communicate with other members of the Hospital and Community care teams in relation to the management of patients, referring to other disciplines as necessary
  • To be able to analyse blood results in order to determine the most appropriate course of action
  • To assist in the development and monitoring of standards to be used for the delivery of patient care, by members of the Community Integrated Diabetes Team, and other professionals in community healthcare settings, and private sector residential care settings

SECTION 1- Main Duties and Responsibilities

  • To assist in the development of policies related to diabetes management for use by all trust clinical staff.
  • To provide a source of information and advice on new advances in diabetes management and control, for other members of the primary healthcare team.
  • To supervise and support less experienced members of the diabetes team in the absence of the team lead
  • To deliver structured education packages to individuals and groups of patients and carers in accordance with Trust requirements.
  • To provide health promotion advice to patients and their carers both formally and informally in a variety of settings
  • Is responsible for following Health and Safety guidelines when dealing with patients and carers on their caseload, within their place of residence or in nurse led clinic settings
  • Maintains accurate records of all nursing interventions and any other activities associated with work, observing Trust Policies, in writing and in electronic format
  • Prioritises own workload, amending plans as necessary in order to accommodate urgent requests
  • Undertakes relevant research as required
  • To initiate and undertake clinical and organisational audit in order to promote high quality nursing practice development in accordance with Trust policy
  • Develops and provides educational programmes for professionals within the Primary healthcare team.
  • Participates in the Performance Management System within the service to meet the changing needs of the Service
  • To attend trust professional, operational and link meetings as appropriate to the role
  • To organise and operate specialist nurse led clinics within defined caseload, communicating information related to patient condition and treatments, with General practice staff, and both internal and external agencies as needed
  • May be required to assess and appropriately prescribe medication and therapy within the boundaries of the services non-medical prescribing formulary, as appropriate to the post holders level of qualification


Person Specification

Qualifications

Essential

  • Registered General Nurse (NMC Level 1)
  • NMC registered
  • Level 6 (Degree) in nursing or health related subject or equivalent experience
  • Evidence of ongoing continuous professional development


Desirable

  • Level 5 Diabetes course or equivalent demonstrable skills
  • Teaching and mentoring qualification or equivalent experience
  • Non-Medical Prescribing qualification


Experience

Essential

  • Post registration experience within related speciality area
  • Demonstrate they have relevant experience and skills working with people with diabetes.


Desirable

  • Experience of teaching in practice or formal lecturing
  • An understanding of DESMOND and DAFNE education programs

  • Evidence of detailed knowledge base in diet therapy, oral and injectable medications for Diabetes.
