Are you a passionate Registered Mental Health Nurse, Occupational Therapist or Social Worker who is keen to make a difference and looking for a new challenge?

This is a great opportunity for you to work in a small multidisciplinary team with a shared caseload as a Band 6 Community Mental Health Professional within Bridgwater Assertive Outreach Team. We are welcoming applications from enthusiastic and creative professionals to join our supportive team.

Main duties of the job

The team provides intensive mental health support to individuals with complex, severe and enduring mental health difficulties in the Bridgwater area, working in a creative and dynamic way to deliver specialised care to patients with complex needs. The team focuses on supporting individuals who historically have struggled to engage with standard community mental health support and are at high risk of relapsing or admission into inpatient services.

You will be working as a senior clinical team member and delivering care for individuals within a shared caseload in the team. You will have a key role in supporting other members of the team in the development and delivery of mental health care in the community.

You will be committed to a recovery approach and have the ability to be flexible and work as part of a team.

You will receive support and guidance from the rest of the team and a clear supervision structure as well as a focus on your own personal and professional development through the Trust's PDR process.

There will also be the opportunity to take on the supervision of junior members of staff within the team under the direction of your line manager.

Job responsibilities

Job Summary

To provide mental health assessment and planned care to service users with serious mental health difficulties. This will involve managing a caseload, multi-disciplinary working and arranging regular reviews of care. This will include, where appropriate, sharing information with carers/relatives and key workers and agencies, and working with them in the process of assessment and the development and implementation of the clients care plan.

Qualifications and Training

Essential

  • Professional registration RMN - Nursing
  • Commitment to Continuing Professional Development


Desirable

  • Evidence of recent relevant training
  • Mentor qualification


Knowledge and Experience

Essential

  • Experience at Band 6 level or 2 years experience in community mental health teams at band 5
  • Experience of care planning


Desirable

  • Working with severe and enduring mental health problems
  • Working with clients who are difficult to engage.


Skills and Abilities

Essential

  • Risk assessment skills
  • Knowledge of current developments in community mental health and assertive outreach
  • Understanding of the Mental Health Act


Desirable

  • Basic IT skills
  • Awareness of Health and Safety issues, particularly concerning lone working
