We are looking for a Physiotherapist to join our small and friendly therapy team in Holderness. If you enjoy working in the community, with a variety of patients, using different types of skills, this may be the post for you. Our team covers both Intermediate Care patients ; rapid response to enable patients to stay at home, and facilitating hospital discharges, and also we have a community rehab caseload, working with patients who have a range of different conditions ( including Neuro, Long Covid, frail elderly, brain injuries )

You will need to be able to work as part of a therapy team ( Physios, Occupational Therapists and support staff) but also must be able to work on your own in the community. This post involves some weekend working and working until 8pm on a Rota pattern.

Main duties of the job

  • To work with a multi-disciplinary team (MDT) delivering services to patients over the age of 18 and are identified as in need of rehabilitation whilst under the care of the Intermediate Care Service.
  • To support the line manager in the delivery of the physiotherapy service provided to their area of work on a day to day basis. To be involved in implementation of any change to work organisation/service delivery.
  • To assess and manage patients within the area of your work who may have complex and / or chronic presentation. To determine clinical diagnosis/need and physiotherapy intervention required. To maintain records as an autonomous practitioner in accordance with legislation and service policy.
  • To be responsible for the designated area of work, as agreed with the physiotherapy team leader, and to plan and organise efficiently and effectively with regard to patient management and use of time.

Person Specification Qualifications Essential
  • Degree in Physiotherapy or equivalent.
  • Current full HCPC Registration
  • Member of special interest group
  • Post-graduate diploma in relevant area
Knowledge Essential
  • Able to justify clinical decisions to other Medical Staff, colleagues and other professionals
  • Competent in skills appropriate to the post e.g. Manual Therapy/Neuro
  • Evidence of CPD activity, including nationally recognised post graduate training in relevant speciality to Diploma level or equivalent
  • Evidence of reflective practice, IST and CPD portfolio
  • Autonomous practitioner works within codes of practice, policies and guidelines.
  • Accountable for own professional action
  • Contributes to the development of policy.
  • Evidence with involvement with clinical governance agenda
Experience Essential
  • Broad based clinical experience covering main speciality areas including community work in relevant specialism
  • Able to work alone/unsupervised.
  • Ability to supervise more junior staff and support staff.
  • Good Time management.
  • Ability to manage own caseload.
  • Able to work in a MD/Multi-Agency setting
  • Participation in audit/evaluation/research.
  • Clinical experience within specialist field
