An opportunity has arisen for an experienced advanced nurse practitioner to join our Community Integrated Response Team, working across urgent community response and frailty virtual ward.

Main duties of the job
The Community Integrated Response Team is going through a very exciting period of transformation.
This post is fundamental for leadership and clinical delivery across the team, and recognises the complexity of the services delivered by the community integrated response team.
Responsible for clinical delivery of services to defined performance indicators including quality standards and patient experience.
You will be working as part of a highly supportive and motivated service
The successful candidate will also work with other clinicians including Physiotherapists, Occupational therapists and social workers, whom are all co-located within the Integrated Gateway. Provide support to the nursing team to ensure that the course of clinical care that is right for the patient.
The services provide care for acutely unwell patients in their own home (inclusive of residential and nursing homes), aiming to avoid unplanned admissions to hospital supporting earlier discharges from hospital and have commenced National Guidance for 2 hr Urgent Care Response
To use established pathways and processes to provide a wide range of nursing and social care assessments to patients who may potentially be at risk of a hospital admission.
We welcome applicants from a range of backgrounds including emergency nurses, primary care and acute medicine as well as nurses from community backgrounds who have experience in working within Integrated Services.
