We are recruiting an experienced professional to lead our hospital flow and discharge arrangements across the Birmingham and Solihull (BSol) health and social care system.

This is an exciting, newly created role, allowing the successful candidate to make a real difference within our system on the back of the creation of an integrated single transfer of care (STOC) team and approach. The successful candidate will have an excellent working knowledge of hospital discharge and admission avoidance policy and best practice. Candidates will need to demonstrate excellent leadership and communication skills, have recent experience of service transformation and be able to take responsibility for the day-to-day oversight and management of the STOC team, ensuring patients receive timely, safe and co-ordinated care.

Main duties of the job

It is fundamental that the BSol Health and Care System creates safe levels of capacity and maintains sufficient flow. This ensures that patients who need inpatient hospital care are admitted and treated efficiently, and then discharged as soon as possible - preferably to their home. The Integrated Single Transfer of Care Lead will be responsible for achieving this. The post requires strong system leadership and establishment and oversight of a regular rhythm of operational challenge, control and support. This will include driving operational teams across the system to focus on the person, co-ordinating and redistributing resources to deliver safe, early discharge and improve patient outcomes. This is a senior manager position, that can make day to day decisions on behalf of all agencies in the system to facilitate discharge. The system is in the process of adopting the principles of 'Discharge to Assess' and 'Home First'.

The postholder will lead the system-level discharge coordinating STOC Hub for the BSol Health and Social Care system, on behalf of University Hospitals Birmingham FT(UHB), Sandwell and West Birmingham Hospitals (SWBH), Birmingham Community Healthcare FT (BCHC), Birmingham and Solihull ICB, Birmingham City Council (BCC) and Solihull Metropolitan Borough Council (SMBC).
