The post holder will work as a member of the Neighbourhood Team in Older Persons' Mental Health providing a high quality, comprehensive mental health service for older people in Taunton and West Somerset with severe and enduring mental health difficulties.
To offer advice, assessment and brief intervention for older adults who have been referred into our service. To provide robust and therapeutic treatment as part of the wider community mental health service response to the population of Somerset.
Main duties of the job
The purpose of the Single Point of Access (SPA) is to assess older people being referred to Taunton and West Somerset OPCMHT to ascertain the most appropriate service to meet their needs. The post requires significant clinical and organisational skills. You will adhere to the goals of 'No wrong Front Door' and support the operations of the Team who seek to identify the right service for a person at the right time.
You will be responsible for coordinating the intake of referrals into Taunton and West Somerset OPCMHT, carrying out telephone triages, face to face assessments and brief interventions with people aged over 65yrs and their families. You will also support the daily management of the duty rota.
The SPA Clinician will work with front line administrators and OPCMHT colleagues to identify presenting needs, manage risk and complete/co-ordinate DUTY visits. This will involve liaising with the person referred, their carers/family members and professionals at the point of referral.

Maintain clear and concise documentation following face to face or telephone appointments with patients and/or their families.
Organise and maintain, alongside admin colleagues, clear referral databases and outcome data for the team.
Responsibility for Patient / Client Care, Treatment & Therapy
The post folder will be expected to offer face to face and telephone appointments to older persons referred to the OPCMHT.
To offer brief assessment and intervention holding a small caseload.
Offer face to face and telephone risk assessments, including creating and planning risk management.
Communicate effectively with First Response service/Intensive Dementia Service where there is an issue of risk.
Where required, ensure patients are effectively safeguarded, including making and liaising with social care where there are identified safeguarding concerns.
Be responsible for the communication about the service to referrers and key stakeholders.
Policy, Service, Research & Development Responsibility.
Where required, working alongside the Team Manager and Neighbourhood HUB team, review and develop the teams policies and procedures.
Where required/appropriate, support the Team Manager and HUB Team in developing the service or carry our service reviews.
Where required/appropriate, engage, participate or lead on QI projects developed for the service.
Responsibility for Finance, Equipment & Other Resources It is the responsibility of all staff that they do not abuse their official position for personal gain, to seek advantage of further private business or other interests in the course of their official duties. You will be responsible for keeping IT and mobile equipment safe secureResponsibility for Supervision, Leadership & Management To receive regular clinical and professional supervision from a senior Clinician and, where appropriate, other senior professional colleagues.
