An exciting opportunity for a dynamic, strategic and enthusiastic individual to join our Community Nursing Team in West Berkshire has arisen. You will be joining a service that has just been recognised as having an 'Outstanding' rating, with the trust also being rated 'Outstanding' in our latest CQC inspection.

In your role you will lead and support staff in order to promote a service which provides complex nursing care to patients in their own homes. You will be responsible for all aspects of the leadership, development and management of designated teams.

Using your skills you will ensure that safe, evidence-based care is delivered to patients, their families and carers in the Community, whilst also being proactive in promoting a positive image of Community Nursing. You will be working closely with the organisation, integrated teams, key stakeholders and partners.

You will be based at 7-9 Cremyll Road, Reading,RG1 8NQ but will be expected to visit other sites as part of your role. The benefit of having core working days of Monday - Friday but with opportunities for flexible working will enable you to ensure that a high-quality service is delivered at all times.

Main duties of the job

  • Responsible for the delivery and operational management of the service.
  • Directing and supporting staff in their clinical roles to promote a service which prevents inappropriate or unscheduled hospital admissions and facilitates timely discharge
  • Responsible for capacity planning and management of a seven day service, liaising with other Team Leaders to
  • ensure equity of staffing and work-load to meet the needs of the service
  • Operationally manage a group of staff, including appraisal and personal development planning review and delivery, performance management, leave and sickness absence, ensuring complete confidentiality is maintained

The4 must haves for you to be considered for this role:
  • Adult nursing qualification along with NMC registration
  • Evidence of clinical leadership within a community nursing environment
  • Evidence of relevant continuous professional development
  • A full UK Driving Licence and have use of a car for work purposes

Education/Qualifications/Training

Essential

  • Registered Nurse - Adult
  • Specialist Practitioner qualification (DN Degree, diploma or Certificate in District Nursing)


Desirable

  • Non-Medical Prescriber /Nurse Prescriber Qualification


Continuous Professional Development

Essential

  • Active on the Mentor register
  • Evidence of Leadership & Management development


Desirable

  • Teaching qualification/or willingness to undertake
  • Recognised coaching qualification or willing to undertake


Additional Requirements

Essential

  • Able to work flexibly across service areas to enhance quality of service provision
  • Valid full UK licence with confidence to drive during unsocial hours and in unusual environments with use of car for work
  • A degree of agility and the ability to manoeuvre light to moderate loads
  • Able to work bank Holidays, weekends and unsocial hours to meet service need
  • Able to work in challenging environments
  • Understanding of QMIS and Quality Improvement initiatives


Previous Experience

Essential

  • Evidence of clinical leadership within a community nursing environment including
  • Teaching/assessing in practice
  • Coaching
  • Recruitment and selection
  • Risk and Incident Reporting (Root Cause Analysis) and supporting teams in learning from findings.
  • Community Nursing experience at Band 6 for 2 years as minimum
  • Able to conduct courageous conversations and support staff with performance related issues
  • Able to support and manage own workload and support others


Knowledge, Skills & Abilities

  • In depth understanding of case co-ordination and the role of Community Nursing
  • Able to motivate self and teams to manage change
  • Works collaboratively with their counterparts from teams in other provider organisations to develop a seamless service
  • Effectively leads, manages and motivates a team by being highly visible, approachable and communicative at all levels utilising effective written, oral and presentation skills.
  • staff to undertake clinical duties implementing evidence based practice

  • Applies policies and procedures effectively to manage risk
  • Manage performance through the application of HR processes
  • Demonstrates flexibility to deal with the needs of unscheduled services and able to prioritise workloads and work under pressure
