Undertaking assessments and coordinating the care of service users. Ensuring that communication with the service users / families / and GP's is of the highest standard. To develop and maintain positive working relationships with service users, their carers, General practitioners, third sector partners, other significant professionals and members of the multi-disciplinary team. Ensuring that clear channels of communication are established and maintained. To promote and identify areas requiring development within the Community Mental Health Team within South East area and to take a lead role in the development of services in conjunction with the Operational Team Manager, head of clinical services and other professionals.

Main duties of the job

1. Manage individual casework in the most efficient manner possible, taking into consideration the economies of time, distance and expense whilst maintaining the highest standards of care.2. Participate in weekly multidisciplinary meetings.3. Maintain recording systems to support the above, providing statistical information to agencies and the Team Manager/head of clinical services as required.5. Delivering professional standards of care to the very highest using evidence based practice.

6. Ensuring that all communications with colleagues, clients and families are clearly understood, giving written confirmation of verbal communications where there is observed to be a need.7. Promote the recovery agenda with service users8.Being conversant with the current educational requirements of learners and assisting in their introduction and supervision whilst attached to the Community Mental Health Services.9. Taking responsibility for maintaining and advancing own professional knowledge and keeping abreast with current developments.10. Participating in research projects of surveys being carried out in the community setting.11. Participating in team development meetings with the Team Manager and other members of the team, using expertise, knowledge and skills.12. Ensuring that professional knowledge is up to date .Displaying motivation within the work place improving skill levels by attending appropriate courses, seminars etc.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment.You are encouraged to enrol for the DBS Update Service. A small annual fee of �13 per year applies.

New entrants to the NHS will commence on the minimum of the scale stated above.

Education / Qualifications

Essential

  • RMN
  • Degree or equivalent level qualification
  • Evidence of continuing professional development
  • Minimum of 2 years post-qualified experience working in a community mental health setting at band 5 or 6


Desirable

  • Mentor qualification
  • Evidence of using Microsoft software


Experience

Essential

  • Experience of implementing significant changes in working practices
  • Experience of monitoring standards of care
  • Experience of providing supervision
  • Experience of working within a multidisciplinary team
  • Working knowledge of dementia
  • Working knowledge of older people with functional disorders


Knowledge

Essential

  • Specialist knowledge relevant to the service
  • Working knowledge of all relevant legislation
