We are currently transforming Mental Health Services in Somerset and have an exciting opportunity for you to be part of an expanding Community Mental Health Service. We are seeking a Mental Health Practitioner (either as a Mental Health Nurse, Occupational Therapist or Social Worker) to join our Community Mental Health Service as part of our support and help line team.
The post holder will work within a Community Mental Health Service that has challenged the traditional boundaries between primary and secondary care in partnership with Service Users and the Voluntary Sector and Community Enterprise.
This post can be undertaken remotely/working from home or if you prefer from our base in Wells Somerset.
Main duties of the job
The post holder will deliver a high quality, comprehensive mental health service for patients with severe and enduring mental health difficulties within the Community Mental Health Service. The role involves a focus on working with individuals who experience a complex range of needs and difficulties.
Acting as a key member of the multi-disciplinary support and help line, the post holder will be required to receive and make calls that offer support over the phone/MS Teams to service users already open to the community mental health service. The post holder will also triage new referrals and/or liaise with referrers, carers, and members of the public to gather information relevant to supporting wider community mental health teams clinical decision-making processes.
The post holder will also be required to undertake assessments using a collaborative, recovery approach and will beskilled at positive risk management. The post holder will contribute and participate in team and service development and evaluation. Liaison within the community mantal health services and a wide range of external agencies is central to the role.
The post holder must hold a full driving licence and/or have access to transport as you will be required to attend trust premises on a need led basis to undertake venue based face to face activities such as team inductions and mandatory training.

- Current registration with relevant professional body
- Significant post registration experience in multi-disciplinary community mental health services
- Good knowledge of resources and working practices across health and social care
- Experience of managing risk of clients in crisis
- Experience and knowledge of working with people with mental health problems
- Excellent communication and interpersonal skills including presentation of cases to multi-disciplinary case conferences.
