An exciting opportunity to Lead and Manage the Proactive Care team. The service started in 2020 and has gone from strength to strength. Supporting service users in the Central Bradford locality the service works proactively to address health inequalities. A large Multi disciplinary team with 11 disciplines employed by both BTHFT and BDCT requires a candidate with excellent Communication skills, a good understanding of the health care agenda and a vision for service development and growth.
Main duties of the job
To lead and manage the Team.
Understand complex health issues and manage the caseload.
Seeknew opportunities and ways of working.
Liaise well with the wider multidisciplinary team.
Work closely with the community and community groups.
Promote anticipatory care, role model proactive working.
