The Mental Health division of Nottinghamshire Healthcare Foundation Trust comprises of three directorates Adult Mental Health, Mental Health Services for Older People and Specialist Services. Main duties of the job

The CRHT team provide intensive home support to service users aged 18 and over, through a range of referral routes. We also work with our community teams and partner organisations to provide a range of support options for people experiencing mental health crisis. The service 'gate keeps' all admissions to acute mental health wards, and our team aims to provide care in the least restrictive manner by offering an alternative to hospital admission through intensive home treatment.

Our team will also support planned admissions to wards when required, facilitate discharges, and work closely with wards to identify patients who could be better supported in their communities.

Mental health nurses in the team have the opportunity to take on additional training and development in any area of the service they are particularly interested in, and all are supported by an excellent mentor and preceptorship service when they first join.

Crisis Resolution Home Treatment

Clinical Nurse Lead/Specialist Prescriber

Position based at Sherwood Oaks Hospital

As part of our current local transformation work Nottinghamshire Healthcare Trust have received investment to develop improved Crisis Home Resolution services in the Mid Notts area. We provide intensive home support to service users aged 18 and over, through a range of referral routes, which include self-referral and work with our community teams and partner organisations to provide a range of support options for people experiencing mental health crisis.

The service gate keeps all admissions to acute mental health wards and our crisis house, and aims to provide care in the least restrictive manner by offering an alternative to hospital admission through intensive home treatment. Where required, the team support planned admission to wards and facilitates timely discharge from inpatient care, working closely with wards to identify patients who could be better supported in their communities.

We are seeking Clinical Nurse Leads/Specialist Prescribers to join our team of Consultant Psychiatrists, Junior Doctors, Non-Medical Prescribers, Nurses, Social Workers and Support Workers providing rapid response for adults who are experiencing an acute psychiatric crisis

Suitable applicants will:

• be a qualified RMN or equivalent qualified health professional with an additional Non- Medical prescribing qualification or be willing to undertake the non medical prescribing Course.
• have experience of working with people with mental health problems in an acute health care/community setting
• be required to assess, the needs of individuals when they are acutely unwell and expected to lead, promote and support a prescribing service as an integral part of the care delivered by CRHT
• act as gatekeeper to all psychiatric admissions and support and manage patient flow through the urgent care system
• have the ability to work as part of a multidisciplinary team, be reliable and motivated
• be able to demonstrate high functioning in risk management
• will be expected to participate in the provision of specialised mental health assessments and developing pharmacological treatment plans as an autonomous practitioner
• in collaboration with the team leader, monitor and lead improvements to standards of care though, supervision of practice, clinical audit, evidenced based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership
• be a compassionate, person-centred individual who is hard working and can be flexible in their approach to helping service users experiencing a mental health crisis
• be committed to delivery of mental health care in the least restrictive environment
• be able to work 24/7 365 days a year rota with the other staff in the team
• hold full UK driving licence with access to a vehicle for business use. Reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
