An exciting opportunity has arisen for a Community Mental Health Nurse to join an established team at Havant Health Centre. Your main responsibilities will include care coordination of service users, Duty Worker on rotation, initial and follow up assessments and supervision and mentorship of junior staff and students.
Our teams are working to make sure there is 'No Wrong Door' for our patients and service users. Our people say that being part of this work is hugely rewarding and energising, that they can work alongside partners to ensure people get the right care in the right place.
We're working collaboratively with patients and families, primary care, social care and the voluntary sector to improve responsiveness and experience, and deliver place-based mental health systems that meet local need.
Due to the nature of this post in order to carry out home visits and attend patient meetings a car driver is essential with access to own car.
The Havant and Waterlooville Community Mental Health Team operates from Havant Health Centre. The team is well known for being approachable and open. We support each other and have developed a culture of honesty and integrity where problems are resolved quickly.
Consideration will be given for the right candidate to be supported in development to Band 6 competencies.
Main duties of the job
The post holder will be based at Havant Health Centre and will need to work flexibly. You will also need to establish good working relationships with the local GPs, Italk service and other community and voluntary organisations. We're investing in our mental health services in South East Hampshire, transforming the way we work to provide outstanding care and building truly rewarding careers.
Your main responsibilities will include care coordination of service users, duty work on rotation, and mental health monitoring and supervision and mentorship of junior staff and students.
The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. We support our staff in developing their clinical skills in attending post graduate training such as physical health and nurse prescribing.
