We are inviting applications for motivated & dynamic Community Matron/aspiring Community Matron to complete/complement our committed team of professionals within North Tees & Hartlepool Foundation Trust's Care Group 1.
Working autonomously as an Advanced Practitioner, the post holder will have responsibility for proactively monitoring & managing patients, recognising the early symptoms of disease exacerbation, acute illness & injuries based on the understanding of long-term conditions, disease processes, current evidence and practice standards.
The service is delivered to a population of patients within the community setting primarily residing in care homes. The key aim of the service is prevention of inappropriate hospital admission or readmission following discharge from acute care. In achieving these outcomes there will be a reliance on comprehensive physical assessment and treatment-planning along-side EHCPs and DNACPR.
Main duties of the job
Working autonomously as an Advanced Practitioner, the post holder will have responsibility for proactively monitoring/managing patients, recognising early symptoms of disease exacerbation, acute illness & injuries based on the understanding of long-term conditions, disease processes, current evidence & practice standards. The service is delivered to a population of patients within the community setting primarily residing in care homes. The key aim of the service is prevention of inappropriate hospital admission/readmission.
Post involves liaising with health/social care providers & other agencies to ensure provision of high quality care management to this group of patients/carers. Post-holders will also forge strong links with the nurse-led unit and community hub which is located on the University Hospital of Hartlepool site delivering pathway based interventions.
The Community Matron service operates over 7 days a week, covering the hours of 08.00-20.00hrs across the Hartlepool and Stockton localities.
