1.0 WTE (37.5hrs) Heart Failure Specialist Nurse
An vacancy has arisen in the specialist cardiology nursing team at ROH hosted by Bury Care Organisation. The heart failure specialist team is dynamic and experienced, supported by a site wide heart failure MDT and Heart failure / Device Cardiologists. This team is integrated, in and out-reaching to patients referred from General Practice with heart failure to support the 5 Oldham clusters and Rochdale Infirmary. This service provides expert advice and treatment to patients with heart failure . The aim is to improve the delivery of evidenced based care to this group of patients.

About us
Applicants will be RGN, registered with the NMC, have a minimum of 5 years post registration experience within a Cardiology speciality. Must drive and have daily use of a car. Experience as a band 7 Heart Failure Specialist nurse is desirable but not essential as this post is also being offered as a training post and welcomes nurses who hold experience within the speciality of cardiology who are interested. The applicants must be highly motivated and focused in developing their knowledge and skills within this specialist area of cardiology at MSc level. The post holder will be required to actively support research and audit. The appropriate support and training will be provided.
As a training post there is an opportunity to develop nurses with existing cardiology experience at band 6 or band 7 who do not meet all the essential criteria but who have shown previous evidence of continual professional development and are willing to undertake training, including MSC modules in heart failure and Non-Medical Prescribing course.

- Manage patient care and adjust treatments within local and national guidelines and evidence base.
- To be actively involved in the development of an integrated chronic heart failure pathway.
- To lead and develop local action plans to meet the supportive and palliative care agenda.
- Work closely with the Multidisciplinary team to deliver effective management of patients suffering chronic heart failure.
- Be responsible for education and training of the multi-disciplinary team in the management of chronic heart failure.
- Provide support across Northern Care Alliance for heart failure education and audit
- Responsible for data collection and completion in order to meet local and national audit requirements for patients suffering chronic heart failure.
- Act in accordance with the NMC Code of Professional Conduct
- Have a responsibility and legal obligation to ensure that information processed for both patients and staff is kept accurate, confidential and secure in line with the Data Protection Act (1998) and the Security and Confidentiality Policy
