Are you a Senior Nurse who is passionate and committed to delivering high quality health care in an Integrated Urgent Care Service environment or an experienced band 5 nurse looking for your first band 6 post?

We have an exciting opportunity to work in our expanding Urgent Community Response Team which supports our Guildford and Waverly patients to stay at home. You will be a key member of our multi-disciplinary team including Advanced Clinical Practitioners (ACPs), Specialist Nurses, Paramedics, Occupational Therapists , Physiotherapists and Associate Practitioners who work together to support patients who are experiencing a health crisis.

Main duties of the job

Working as a team player you will work both independently and collaboratively with staff from all disciplines. You will maintain high professional standards and achieve best outcomes for our patients.

You will provide assessments to investigate, diagnose and treat patients in their own environment and prevent unnecessary hospital admissions. You will take responsibility for the short-term case management of your patients whilst on the Urgent Community Response caseload and will identify and facilitate a safe discharge to ongoing services to ensure seamless, joined up care .

The Urgent Community Response Service is a 7-day service and operates from 08.00 - 20.00hrs. You would be expected to be able to cover all aspects of the shift pattern.

You will receive ongoing clinical support, education, and supervision and will work flexibly in response to community service needs.

Job responsibilities

The Urgent community Response Team are based in the Community Co Ordination Centre triaging referrals from GPs, Paramedics on scene, A& E colleagues & other health & social care colleagues. You will be expected to visit patients that require an urgent holistic assessment in order to prevent hospital admission. These may be patients already known to Adult Community Services who have had a change in their care/nursing needs or new patients referred in from external services.

Qualifications

Essential

  • NMC registered Nursing Qualification & evidence of Continued Professional Development (CPD)


Desirable

  • Physical Assessment at Level 6 or 7 or a desire to Complete this as CPD.
  • Mentorship qualification


knowledge and skills

Desirable

  • Some understanding in differential Diagnosis / Physical assessment in order to formulate an impression as to why a patient may have deteriorated
  • Has an understanding of the background to and aims of current healthcare policy/national guidance/CQC/ and appreciate the implications of this on engagement
  • Has some understanding of Adult community services
  • Has experience working as part of a multidisciplinary team
