An opportunity has arisen for an experienced Nurse / Therapist to join an established, progressive and developing Oldham Integrated Discharge Team for an 18 month period.

You should be able to demonstrate the skills and knowledge of working within a multidisciplinary team setting, but equally have ability to work to your own initiative. Working flexibly between Health and Social Care you will provide specialist assessments for Home first , Discharge to Access bed base placements including intermediate Care & Palliative discharges on the Oldham site.

You will have experience and knowledge of community nursing, evidence of carrying out complex assessments and the management of a highly complex caseload, facilitating safe and timely discharge from a hospital setting and have knowledge and experience in dealing with palliative and end of life discharges. You will have the ability to effectively assess, plan, prioritise and evaluate care and workload efficiently, using resources appropriately. Deputise in your managers absence.

Dealing with rapid discharge and changes in relation to Covid-19.

Main duties of the job

The integrated Discharge team work collaboratively with the residential and Nursing home providers providing a trusted assessor service to reduce delays and improve patient flow. This will be an integral part of the role.

Qualifications

  • Registered Nurse / AHP - pin up to date


Desirable

  • Degree level study


Experience

Essential

  • Substantial post registration experience in an acute / community setting


Desirable

  • Experience of discharge co-ordination(facilitation)
  • Community nursing experience


Knowledge

Essential

  • Evidence of relevant Continuing Professional Development
  • Knowledge of Discharge policy , DOH guidance and legislation.
  • Knowledge and experience of the co-ordination of complex discharges including home first, D2A and EOL
  • Clear knowledge of the discharge process
