An exciting opportunity has arisen for a Community Physiotherapist to join the newly established Integrated Community Rehabilitation Service in Central Manchester. This post provides a unique opportunity to use your Specialist Physiotherapy Rehabilitation skills delivering high quality care to patients in their own homes and clinic settings. This is a 30 hour post which offers you the flexibility for work life balance, and has been created following retirement of a current team member.
We are looking for a Physiotherapist who has a wide variety of rotational experience, ideally including respiratory and community rehabilitation, and is also keen to develop their skills further whilst contributing to service development. You will be supported by the Team Lead/ Specialist Physiotherapist and will have excellent peer support from the wider MDT team members.
If you are an enthusiastic, well-motivated HCPC registered physiotherapist with an interest in working with patients with a variety of conditions, we are looking for you to join our community rehabilitation team. UK driving license and access to own vehicle are essential for the post.
Main duties of the job
Assessment, treatment and management of patients with wide variety of health conditions, using clinical reasoning and a variety of evidence-based skills.
Working in a variety of settings including patients own homes, clinics and residential homes as well as holding virtual consultations.
To be responsible for your own clinical caseload completing initial assessments, appropriate goal setting and interventions to address patients' rehabilitation needs.
To work as part of the wider Community Rehabilitation Services to meet the demands of the services in other areas.
To support the Team Lead in the development of the service through keeping abreast of evolving research and evidence-based practice, and ensuring equity of services citywide.
To supervise the Rehabilitation Assistant to support with individualised treatment programmes.
Good IT, written and verbal communication skills are important as the post holder will be required to liaise with a wide range of health care professionals, patients and carers.
Flexible approach to diary and caseload management, often working autonomously.
Regular liaison with wider team and health and social care professionals.
The individual will take responsibility for the education of students.
Due to the nature of the role it is essential that applicants can drive and have access to a car for business purposes.
