An exciting opportunity has arisen for a Mental Health Nurse to work within the HPFT Integrated Care Team within the Stevenage area This is a unique position for someone who enjoys teamwork, challenges and variety in their day to day work. The nurse will be working collaboratively with other health colleagues with service users who have complex frailty needs that are impacting on their mental wellbeing.

Main duties of the job

You will be working in the Stevenage area supporting our service users with mental health and wellbeing. in heir own home, including nursing and residential homes. You will be working in primary care alongside community matrons, occupational therapists, physiotherapist, social workers, and other professionals based within an integrated community team. You will be part of an wider supportive and holistic multidisciplinary team of mental health nurses, mental health OT's and Psychology team.

Job description

Job responsibilities

You will visit service users who are identified under the frailty pathway, in their place of residence, to complete a holistic assessment of their physical and mental health needs. You will work in partnership with other healthcare professionals and trusts. Integrated Care Teams aim to make healthcare more preventative, this service supports service users with multiple long term or complex conditions to remain at home rather than going into hospital or residential care. Referrals are accepted from primary care, ambulance service, A& E and secondary care.

Physical Skills

Essential

  • Computer literacy skills in order to enter data onto electronic client records
  • Car driver (unless you have disability as defined by the Equality Act 2010 which prevents you from driving)
  • Aility to complete tasks associated with the job (subject to Occupational health)


Physical effort

Essential

  • Work will be undertkaen in office base, client homes and potentially some remote working
  • A high degree of travel is required


Experience

Essential

  • Knowledge of legislation related to mental health and social care (e.g. Mental Health Act, Mental Capacity Act, Care Act, Care Programme Approach etc)
  • Good knowledge of Microsoft Word, PowerPoint and Excel.
  • Ability to manage distressing and challenging situations


Desirable

  • Supervision Skills
  • Leadership Skills


Knowledge

Essential

  • Degree, Diploma, or equivalent in Nursing
  • Registration with Nursing and Midwifery Council
  • Mentorship Preparation Course.
  • Two years post registration experience
  • At least one year post graduation experience in Older Adult Mental Health Care.


Desirable

  • Pre/Post registration training/short courses in mental health
  • Community and Inpatient nursing experience
  • AMHP/NMP qualification
  • Physical healthcare training
  • Experience of working with clients with delirium
  • Tier 2 dementia training
  • Experience of working in primary care settings

  • Excellent communication skills, verbally and in writing
  • Ability to feedback and discuss cases within a multi-disciplinary team
  • Excellent report writing skills
  • Excellent documentation of duties performed
  • Confidence to work autonomously in the community
