We are able to offer a fantastic opportunity to join the Diabetes specialist nursing team. We are seeking to appoint a highly motivated staff nurse to work within our current diabetes team as a Specialist nurse. This is a demanding role requiring commitment, enthusiasm and a desire for self development. Previous experience in the care and management of diabetes is preferred, however you will be given full training and support to complete our inhouse training programmes, external training opportunities will be offered where appropriate. The unit has a positive attitude towards training and professional development.
This role will primarily involve the educational support of patients with diabetes related conditions, education of trust staff regarding these conditions and clinical support of the ward teams with some support in delivering specialist clinics.
The service currently holds 1.5 WTE vacancies and are looking to recruit nurses to support the team. The post holder must be able to work alternate sites according to service needs.
Main duties of the job
To assess, plan and implement and evaluate evidence-based nursing care for people with complex needs for their Diabetes management. The post holder is required to provide expert clinical care for patients and to support other staff to develop their competencies in diabetes management. The post holder will be involved in teaching and training both formally to clinical staff and to patients. The post holder will be required to work within Shrewsbury and Telford Acute Trust, however, where appropriate to work collaboratively with primary and community care, and link to other specialists within the acute setting to implement the integrated care pathway for people with diabetes. The post holder may be required to support with collection of data for clinical audit, service development and quality improvement work.
