Clinical Nurse Lead - 37.5 hours per week
An exciting and challenging opportunity has arisen for dynamic and enthusiastic Clinical Lead Nurse to join the Brighton South community nursing team. The focus of this role is to provide safe, effective and quality senior clinical care, along with strong clinical leadership, advice and support to the team.
PCNs are groups of practices working together in collaboration with community and other health and care services. PCNs will enable greater provision of proactive, personalised, coordinated and more integrated health and social care.
Care is organised around individuals' identified needs through collaborative working by service providers, enabling us to deliver excellent patient care at the heart of the community.
Main duties of the job
This post of Clinical Nurse Lead is for an experienced and qualified community specialist practitioner who has advanced practice qualifications including clinical and leadership skills.
You will be expected to coordinate complex care and assessments and management of long term conditions, and to reduce hospital admissions by providing robust contingency plans. Case management skills are also required, to enable self-management of health, and independent living in the home/community setting.
You will need to provide training and support to junior members of the team and deputise for the Team Lead when necessary.

The post holder will also be responsible for staff management, supervision and development, including appraisals and supervision of junior staff. As an educator, mentor and preceptor, you will take the lead in development and supervision, particularly for Community Nurses, as well as promoting self-care management principles with patients using innovative patient activation strategies.
Excellent communication and negotiation skills are essential, as well as an ability to challenge practice as needed.
